目的探讨超声引导下细针穿刺制作细胞块术前诊断涎腺恶性肿瘤的应用价值。方法在彩色多普勒超声引导下,对229个涎腺肿块进行细针穿刺,穿刺标本制成细胞块,做出细胞病理学诊断。结果超声定位准确度为100%(229/229);对照术后病理组织学结果,超声对涎腺恶性肿瘤病理分型诊断准确度为16.2%(12/74);细胞病理学对涎腺肿块病理分型诊断准确度为95.6%(219/229),对涎腺恶性肿瘤病理分型准确度为94.6%(70/74),显著高于超声诊断(P<0.01)。结论超声对涎腺肿块定位准确度高,引导细针穿刺制作细胞块在术前诊断涎腺恶性肿瘤方面具有重要意义。
Objective To investigate the value of ultrasound-guided fine needle aspiration in the diagnosis of salivary gland malignant tumors. Methods Fine needle aspiration was performed on 229 salivary glands under the guidance of color Doppler ultrasound. The punctured specimens were made into cell masses to make cytopathological diagnosis. Results The accuracy of ultrasound localization was 100% (229/229). The histopathological results showed that the diagnostic accuracy of ultrasound was 16.2% (12/74) in the pathological classification of salivary gland malignant tumors. The diagnostic accuracy of pathological typing was 95.6% (219/229). The accuracy of pathological classification of salivary gland malignant tumors was 94.6% (70/74), which was significantly higher than that of ultrasound (P <0.01). Conclusion Ultrasound has a high accuracy in the localization of salivary gland masses. It is of great significance to guide the fine-needle aspiration in the diagnosis of salivary gland malignant tumors.
