目的观察不同阶段糖尿病肾病(DN)患者血清中chemerin的水平变化,探讨其与DN的相关性。方法 2型糖尿病(T2DM)患者104例分为正常蛋白尿组(NUAlb组)、微量蛋白尿组(MUAlb组)及大量蛋白尿组(CUAlb组),对照组(NC组)为28名健康体检者。应用酶联免疫分析法(ELISA)测定血清chemerin。结果 CUAlb组血清chemerin水平较NC组、NUAlb组及MUAlb组明显升高(P﹤0.01),chemerin水平与病程、BMI、SBP、TG、FPG、HOMA-IR、HbA1c、BUN、Cr均呈正相关(P﹤0.01),与Ccr、HDL-C呈负相关(P﹤0.01)。线性回归分析示SBP、BUN、Ccr及HDL-C与血清chemerin水平关系密切。结论 2型糖尿病肾病患者血清chemerin水平升高可能预示肾功能减退及糖尿病微血管病变的进展。
Objective To investigate the changes of serum chemerin levels in patients with diabetic nephropathy (DN) at different stages and to explore its correlation with DN. Methods One hundred and four patients with type 2 diabetes mellitus (T2DM) were divided into normal proteinuria group (NUAlb group), microalbuminuria group (MUAlb group) and massive proteinuria group (CUAlb group), and control group (NC group) By. Serum chemerin was determined by enzyme-linked immunosorbent assay (ELISA). Results The level of chemerin in CUAlb group was significantly higher than that in NC group, NUAlb group and MUAlb group (P <0.01). There was a positive correlation between chemerin level and duration of disease, BMI, SBP, TG, FPG, HOMA-IR, P <0.01), but negatively correlated with Ccr and HDL-C (P <0.01). Linear regression analysis showed that SBP, BUN, Ccr and HDL-C were closely related to serum chemerin levels. Conclusions Elevated chemerin level in patients with type 2 diabetic nephropathy may predict the progression of renal dysfunction and diabetic microangiopathy.
