目的探讨双重血浆置换(DFPP)清除抗肾小球基底膜抗体效果。方法选择2010年2月至2013年10月采用DFPP治疗的I型肾小球肾炎男性患者8例,对比分析治疗前后GBM抗体浓度、白蛋白、Ig G和纤维蛋白原(FIB)的水平。结果治疗后GBM抗体浓度呈线性下降,下降斜率(-20.3±1.1)RU/ml/次。FIB下降平均值(49.2±7.9)%,而Ig G水平下降程度可以用指数形式表示。结论采用双重血浆置换清除可以同时有效清除Ig G和GBM抗体,还可清除纤维蛋白原。
Objective To investigate the effect of double-plasmapheresis (DFPP) on anti-glomerular basement membrane antibody. Methods Eight male patients with type I glomerulonephritis treated with DFPP from February 2010 to October 2013 were enrolled. The levels of GBM antibody, albumin, Ig G and fibrinogen (FIB) were compared before and after treatment. Results After treatment, the concentration of GBM antibody decreased linearly with decreasing slope (-20.3 ± 1.1) RU / ml / time. FIB decreased mean (49.2 ± 7.9)%, while the level of Ig G decline can be expressed in exponential form. Conclusion Double plasmapheresis can effectively remove Ig G and GBM antibodies and clear fibrinogen simultaneously.
