目的:探讨大孔树脂和活性炭成分的两种灌流器对血液灌流患者血小板的影响。方法:将我科2013年1月-2014年12月收治的行血液灌流的急性中毒患者54例随机分为大孔树脂组和活性炭组,每组27例,大孔树脂组采用MG350大孔树脂成分灌流器,活性炭组采用Adsorba300C活性炭成分灌流器,每次作床边血液灌流2.5小时,共两次,比较两组患者灌流后血小板的数量。结果:活性炭组灌流一次后,27例(100%)患者血小板明显下降,灌流第二次后血小板下降更加显著,其中有6例要输注血小板后才可以继续治疗,有3例被迫停止血液灌流。大孔树脂组有24例(89%)患者有不同程度的血小板减少,但下降速度较慢,不影响继续治疗。结论:用活性炭成分的灌流器行血液灌流后血小板的下降数比用大孔树脂成分灌流器血液灌流后明显。
Objective: To investigate the effects of two kinds of perfusate with macroporous resin and activated carbon on platelet in hemoperfusion patients. Methods: 54 cases of acute hemoperfusion patients admitted from January 2013 to December 2014 in our department were randomly divided into macroporous resin group and activated carbon group, with 27 cases in each group. The macroporous resin group was prepared with MG350 macroporous resin The components of perfusion, activated carbon group using Adsorba300C activated carbon component perfusion, each bedside blood perfusion 2.5 hours, a total of two times, comparing the two groups of patients after perfusion of platelets. Results: In the activated carbon group, the platelet count of 27 patients (100%) decreased significantly after perfusion, and the second time after perfusion, the decrease of platelet count was more significant. Six patients were allowed to continue treatment after transfusion of platelets, and 3 patients were forced to stop the blood Perfusion. In the macroporous resin group, 24 patients (89%) had different degrees of thrombocytopenia, but the decline rate was slow, which did not affect the further treatment. CONCLUSIONS: The decrease in platelet count after perfusion with activated charcoal cartridges was significantly greater than that after perfusion with macroporous resin cartridge.
