Germinoma is the most common type of germ-cell tumor. Extragonadal germ cell tumors are frequently situated in the midline structures of the body, such as the anterior mediastinum, retroperitoneum, and the brain. Primary intramedullary spinal cord germinoma is very rare. We report a case of primary germinoma originating from the intramedullary portion of the conus medullaris.
