目的:探讨宫颈环形电切术(LEEP)及微波治疗Ⅲ°宫颈糜烂的临床效果。方法:选择Ⅲ°宫颈糜烂患者120例,随机将其分为两组,治疗组采用LEEP治疗;对照组采用传统的微波治疗,每组各60例。评定两组患者术中出血量、术后15 d脱痂止血及治疗后60 d治愈、好转和无效情况,治疗后90 dHPV高危型持续感染率。结果:LEEP60例(其中HPV感染20例),术后15 d脱痂止血2例,术后60、90 d复查,60例宫颈创面100%愈合(愈合率100%);术后90 d复查HPV高危型持续感染2例(10%),对照组60例(其中HPV感染18例),术后15 d脱痂止血12例,术后60 d复查,宫颈创面完全愈合42人(愈合率70%),18例创面愈合1/3~1/2,4例完全未愈合;术后90 d复查,HPV高危型持续感染6例(30%)。两组比较差异具有统计学意义(P<0.05)。结论:LEEP治疗宫颈Ⅲ°糜烂,具有创面愈合时间短,HPV持续感染率低的优点,其疗效优于传统微波治疗。故重度宫颈糜烂可首选LEEP治疗。
Objective: To investigate the clinical effect of cervical ring electrosurgical excision (LEEP) and microwave treatment of Ⅲ ° cervical erosion. Methods: 120 patients with Ⅲ ° cervical erosion were selected and randomly divided into two groups. The treatment group was treated with LEEP. The control group received conventional microwave treatment of 60 cases in each group. Blood loss was assessed in both groups, 15 days after operation, escaping bleeding and 60 days after treatment were cured, improved and ineffective, 90 days after treatment of high-risk type of persistent infection. Results: There were 60 cases of LEEP (including 20 cases of HPV infection), 2 cases of esophageal squamous cell carcinoma after 15 days of operation, 60 cases of 90 days after operation, 60 cases of cervical wound healing (100%); 90 days after operation Two cases (10%) were infected with high-risk type, 60 cases (HPV infection was 18 cases) in the control group, 12 cases were treated with desquamation and hemostasis after operation, 60 days after operation, 42 cases were completely healed (the healing rate was 70% ), 18 cases of wound healing 1/3 ~ 1/2, 4 cases did not heal completely; 90 days after the operation, HPV infection in 6 cases (30%). The difference between the two groups was statistically significant (P <0.05). Conclusion: LEEP treatment of Ⅲ ° erosion of the cervix, with a wound healing time is short, HPV infection rate of the advantages of low, the effect is better than the traditional microwave treatment. Therefore, severe cervical erosion can be the preferred LEEP treatment.
