目的 探讨国产复方口服避孕药 (COC)与凝血因子的变化及因子V基因突变的关系。方法 测定 14 1例健康妇女凝血因子Ⅱ、Ⅴ、Ⅶ、Ⅷ、Ⅹ活性 ;分析女性脑卒中患者因子Ⅴ基因多态性。结果 炔诺酮组和炔诺孕酮组凝血因子X活性均数均显著高于对照组 (97 0 8± 14 78,P <0 0 5 ;98 5 8± 13 2 8,P <0 0 1) ;炔诺酮组凝血因子Ⅷ活性均显著高于对照组 (15 0 96± 5 7 11,P <0 0 5 ) ;二服药组凝血因子Ⅱ、Ⅴ、Ⅶ活性均数与对照组相似 ,差异无显著性 ;女性脑卒中患者中服避孕药者和非服避孕药者均未发现因子Ⅴ基因多态性。结论 长期服用国产低剂量COC对妇女的凝血因子有一些负面影响 ,不同避孕药的影响略有不同 ;COC不会引起中国妇女凝血因子ⅤLeiden突变。
Objective To investigate the relationship between domestic compound oral contraceptives (COC) and coagulation factors and the mutation of factor V gene. Methods The activity of coagulation factors Ⅱ, Ⅴ, Ⅶ, Ⅷ and Ⅹ in 14 healthy women were determined. The gene Ⅴ gene polymorphism in female stroke patients was analyzed. Results The norethindrone group and norethindrone group were significantly higher than the control group (97 0 8 ± 14 78, P <0 05; 98 58 ± 13 2 8, P 0 01 ); Norethindrone group coagulation factor Ⅷ activity were significantly higher than the control group (15 0 96 ± 5 7 11, P <0 05); two medication group coagulation factor Ⅶ, Ⅴ, Ⅶ activity similar to the control group, There was no significant difference between the two groups. No factor Ⅴ gene polymorphism was found in women with stroke and non-contraceptives. Conclusion Long-term use of domestic low-dose COC has some negative effects on clotting factors in women. The effects of different contraceptives are slightly different. COC does not cause the mutation of coagulation factor ⅤLeiden in Chinese women.
