目的:探究HPV(人乳头瘤病毒)检测在宫颈癌病变检查中的临床应用价值。方法:对2012年-2015年期间到我院检查的2970例疑似宫颈癌患者的档案进行回顾性分析,分别使用TCT细胞检测法和HPV DNA检测法对其进行检查,二者中若有一方结果呈阳性,则需要采取阴道镜活检的方法,对其进行确诊,以确保得到患者准确的患病情况。结果:经过病理学鉴定,2970名疑似患者中共有宫颈癌变患者148人,HPV结果呈阳性患者共有501人,HPV感染率为16.87%,不同类型的宫颈癌变患者的HPV感染率有所差别,CIN(宫颈上皮内瘤变)的三个级别中,CINⅠ的HPV感染率为81.3%,CINⅡ的HPV感染率为85.7%,CINⅢ的HPV感染率为95.4%,浸润癌的HPV感染率为83.4%,经过数据分析统计,HPV检测宫颈癌变的准确率为88.9%;TCT结果呈阳性患者共有378人,宫颈癌变检测准确率为75.76%。两种检测方法的数据比较有显著性差异(P<0.05)。结论:HPV检测在不同级别的CIN准确率有所差别,在宫颈癌变的检测中准确率明显高于TCT细胞检测,说明HPV在宫颈癌变的检测中有较好的临床应用价值。
Objective: To investigate the clinical value of HPV (human papillomavirus) test in cervical cancer lesions. Methods: The archives of 2,970 suspected cervical cancer patients who were admitted to our hospital from 2012 to 2015 were retrospectively analyzed. The results of TCT cell test and HPV DNA test were used to examine them. Positive, you need to take a colposcopy biopsy method, its diagnosis, to ensure that patients get the exact prevalence. Results: After the pathological identification, there were 148 out of 2970 suspected patients with cervical cancer, the total number of HPV positive patients was 501, the HPV infection rate was 16.87%. The HPV infection rates of different types of cervical cancer patients were different, CIN (Cervical intraepithelial neoplasia), the HPV infection rate of CINⅠwas 81.3%, HPV infection rate of CINⅡwas 85.7%, HPV infection rate of CINⅢ was 95.4%, invasive HPV infection rate was 83.4% After data analysis and statistics, the accuracy of HPV detection of cervical cancer was 88.9%; 378 patients with positive TCT results, cervical cancer detection rate was 75.76%. There was significant difference between the two methods (P <0.05). Conclusion: The accuracy of HPV test in different levels of CIN is different. The accuracy of HPV test in cervical cancer detection is obviously higher than that of TCT cell test, indicating that HPV has a good clinical value in the detection of cervical cancer.
