目的:探讨海南省黎族孕妇α-地中海贫血和铁蛋白缺乏的发生率,以及与红细胞相关参数的关系。方法:收集海南省保亭和琼中两个地区黎族孕妇EDTA-K2抗凝全血和血清标本各115例进行血常规检测,同时用Gap-PCR技术进行3种常见α-地贫缺失基因分析,用化学发光法对其血清进行铁蛋白检测。结果:115例海南黎族孕妇中,α-地贫患者62例,占54.39%;铁蛋白缺乏59例,占51.30%。α-地贫合并铁蛋白缺乏占29.57%。α-地贫以静止型为主,占40.35%。静止型α-地贫与正常值相比,MCV、MCH和RDW值差异无统计学意义(P>0.05),标准型和中间型α-地贫与正常对照相比,MCV、MCH和RDW值差异均有统计学意义(P<0.05)。单纯铁蛋白缺乏MCV、MCH和RDW值基本正常,单纯α-地贫和α-地贫伴铁蛋白缺乏MCV和MCH均明显降低,RDW基本正常。单纯α-地贫和α-地贫伴铁蛋白缺乏比较差异无统计学意义(P>0.05)。115例海南黎族孕妇标本中共检出小细胞低色素性贫血41例,以单纯α-地贫和α-地贫合并铁蛋白缺乏为主,各组疾病类型间MCV、MCH和RDW值差异无统计学意义(P>0.05)。结论:海南黎族孕妇α-地中海贫血及α-地中海贫血伴铁蛋白缺乏的发生率非常高,分析红细胞相关参数对其临床诊断有一定的帮助。
Objective: To investigate the incidence of α-thalassemia and ferritin deficiency in pregnant women of Li ethnic group in Hainan Province and their relationship with erythrocyte related parameters. Methods: The blood samples of 115 pregnant women with EDTA-K2 anticoagulant blood from Baoting and Qiongzhong were collected and analyzed by Gap-PCR. , The chemiluminescence method for the detection of ferritin in its serum. Results: Of the 115 Hainan pregnant women with Li-thalassemia, 62 cases were a-thalassemia, accounting for 54.39%; the lack of ferritin was 59 (51.30%). α-thalassemia with ferritin deficiency accounted for 29.57%. α-Thalassemia mainly static, accounting for 40.35%. There was no significant difference in MCV, MCH and RDW between the resting α-thalassemia and the normal (P> 0.05). MCV, MCH and RDW values of the normal and intermediate α-thalassemia patients The differences were statistically significant (P <0.05). Hypofunctional ferritin lacked MCV, MCH and RDW values were basically normal, and both pure a-thalassemia and a-thalassemia ferritin lacked MCV and MCH, while RDW was almost normal. There was no significant difference between pure alpha-thalassemia and alpha-thalassemia with ferritin (P> 0.05). A total of 41 cases of microcytic hypochromic anemia were detected in 115 cases of pregnant women from Hainan Li ethnicity. The prevalence of a-thalassemia and a-thalassemia combined with ferritin deficiency was the main one. There was no statistical difference in MCV, MCH and RDW between the groups Significance (P> 0.05). Conclusion: The prevalence of α-thalassemia and α-thalassemia with ferritin deficiency in pregnant women of Hainan is very high. It is helpful to analyze the related parameters of erythrocytes in clinical diagnosis.
