目的观察原发性高血压(EH)和其合并T2DM患者血压节律及心脏结构功能的特点。方法随机选择EH组44例,EH合并T2DM(EH&T2DM)组50例,采集临床指标进行统计分析。结果 EH&T2DM组舒张末期左室内径(LV)和左室重量(LVM)较EH组升高,夜间收缩压下降率(SB-PF)、左室舒张早期与晚期二尖瓣最大血流速比值(E/A)和左室射血分数(LVEF)较EH组降低。EH&T2DM组血压非杓型变化发生率较EH组升高。非杓型变化的患者较杓型变化患者均出现LV增大,LVM增加。所有患者SBPF与舒张末期室间隔厚度(IVS)、LVM、LV呈负相关。结论EH&T2DM时容易出现血压昼夜节律异常,对心脏结构和功能的影响更为明显。血压昼夜节律异常可能是高血压患者心脏结构和功能变化的主要原因之一。
Objective To observe the characteristics of blood pressure rhythm and cardiac structure and function in patients with essential hypertension (EH) and its combined with T2DM. Methods Forty-four EH patients and 50 EH T2DM patients were randomly selected. The clinical data were collected and analyzed statistically. Results Compared with EH group, the left ventricular diameter (LV) and left ventricular mass (LVM) at end-diastole of EH & T2DM group were increased, SB-PF at night and ratio of maximum blood flow to early mitral valve E / A) and left ventricular ejection fraction (LVEF) decreased compared with EH group. The incidence of non-dipper changes in EH & T2DM group was higher than that in EH group. Patients with non-dipper changes compared with dipper patients showed increased LV, LVM increased. All patients SBPF and end-diastolic ventricular septal thickness (IVS), LVM, LV was negatively correlated. Conclusion EH & T2DM are prone to abnormal circadian rhythm of blood pressure, which has a more obvious effect on cardiac structure and function. Abnormal circadian rhythm of blood pressure may be one of the main causes of cardiac structural and functional changes in hypertensive patients.
