目的:比较第六版和第七版AJCC分期在预测食管癌预后中的作用。方法:回顾性分析150例食管癌患者的临床资料。采用单因素和多因素分析分别评估第六版和第七版AJCC分期中的临床病理特征对预后的预测价值。结果:单因素分析中,第六版和第七版AJCC pT分期和pTNM分期均对预后有显著影响,P均<0.01;第六版pN分期生存曲线分开明显,第七版pN分期的生存曲线部分重叠,pN分期P均<0.01。多因素分析中,第六版和第七版AJCC pT分期P分别<0.05和<0.01,pN分期P分别<0.05和<0.01。单因素和多因素分析中第六版M分期、第七版肿瘤分级和肿块位置均与预后无关;但是将第七版肿块位置重新组合后发现在单因素和多因素分析中,上端食管癌较中段联合下段食管癌预后差,P均<0.05。结论:第六版和第七版AICC分期中pT分期均能预测预后,pN分期以第六版占优势,pM分期以第七版更占优势。第七版AJCC分期中的肿瘤分级与预后无关,第七版AJCC分期中的肿块按照上段和中段+下段来划分则具有预测作用。
OBJECTIVE: To compare the role of AJCC staging in the sixth and seventh editions in predicting the prognosis of esophageal cancer. Methods: A retrospective analysis of 150 cases of esophageal cancer in patients with clinical data. Univariate and multivariate analyzes were used to assess the prognostic value of the clinicopathological features in the sixth and seventh editions of the AJCC staging, respectively. Results: In the univariate analysis, both the sixth and seventh editions of AJCC pTNM staging and pTNM staging had a significant effect on prognosis (P <0.01). The sixth edition of pN stage survival curve was significantly separated, the seventh edition of pN stage survival curve Partial overlap, pN staging P <0.01. In the multivariate analysis, the sixth and seventh AJCC pT stages were <0.05 and <0.01, respectively, and the pN stage was <0.05 and <0.01, respectively. Univariate and multivariate analysis of the sixth edition of M staging, seventh edition of tumor grade and lump location have nothing to do with the prognosis; but the seventh edition of the lump location recombination found in univariate and multivariate analysis, the upper esophageal cancer than The middle section of lower esophageal cancer with poor prognosis, P <0.05. CONCLUSION: The prognosis of pT staging in the sixth and seventh editions of AICC staging can all predict the prognosis. The pN staging is dominated by the sixth edition and the pM staging is more dominant than the seventh edition. The seventh edition of AJCC staging tumor grade has nothing to do with the prognosis, the seventh edition of AJCC staging in accordance with the upper and middle + lower segment to have a predictive role.
