目的:观察系统性红斑狼疮(SLE)患者甲状腺激素水平改变。方法:选择SLE 75例,分为抗双链DNA抗体与抗核小体抗体双阳性组、非双阳性组,另选择健康志愿者70例为对照组。比较其血清三碘甲状腺原氨酸(T3)、甲状腺素(T4)、游离T3(FT3)、游离T4(FT4)和促甲状腺素(TSH)水平,以及甲状腺球蛋白抗体(TGAb)和甲状腺微粒体抗体(TMAb)阳性率。结果:SLE患者总体T3、T4、FT3、FT4水平均显著低于对照组(P<0.05);抗双链DNA抗体与抗核小体抗体双阳性者T3、T4、FT3、FT4水平显著或非常显著低于对照组(P<0.05,P<0.01),也显著低于非双阳性组(P<0.05);TSH水平各组之间均差异不显著(P>0.05)。抗双链抗体和抗核小体抗体双阳性组甲状腺自身抗体阳性率显著高于非双阳性组(P<0.05)。结论:SLE患者甲状腺激素降低,抗双链DNA抗体与抗核小体抗体阳性者降低更显著。 Objective: To observe the changes of thyroid hormones in patients with systemic lupus erythematosus (SLE). Methods: Seventy-five patients with SLE were divided into two groups: anti-double-stranded DNA antibody and anti-nucleosome antibody double positive group and non-double positive group. Another 70 healthy volunteers were selected as control group. Thyrotropin (T3), thyroxine T4 (T4), free T3 (FT3), free T4 (FT4) and thyrotropin (TSH) were measured and serum thyroglobulin antibody (TGAb) Antibody (TMAb) positive rate. Results: The levels of T3, T4, FT3 and FT4 in patients with SLE were significantly lower than those in controls (P <0.05). The levels of T3, T4, FT3 and FT4 in patients with SLE were significantly or very significantly higher than those in patients with double positive (P <0.05, P <0.01), but also significantly lower than those in non-double-positive group (P <0.05). There was no significant difference in TSH levels between groups (P> 0.05). The positive rates of thyroid autoantibodies between double-positive anti-double chain antibody and anti-nucleosome antibody positive group were significantly higher than that of non-double positive group (P <0.05). Conclusions: Thyroid hormone is reduced in SLE patients, and the anti-double-stranded DNA antibodies and anti-nucleosome antibodies are more significantly decreased.
