目的调查分析兰州市汉族成人骨强度指数特点及其与体成分之间的关系,为该地区骨质疏松的预防和干预提供参考依据。方法采用超声骨密度仪及生物电阻抗分析仪测量2291例(男1021例,女1270例)兰州市汉族成人右足跟骨骨强度指数及体成分等相关指标,采用多元逐步回归法分析骨强度指数与体成分各指标之间的关系。结果兰州市汉族成人骨强度指数随年龄增长而持续降低,峰值骨量出现在20~30岁。其中男性20~30岁和50~60岁两个年龄段骨强度指数下降较快,而女性50岁后骨强度指数迅速下降;兰州市汉族男性各年龄组骨强度指数均高于同龄组女性。与采用相同方法测量其他民族、地区人群有关指标进行比较得知,兰州市汉族成人骨强度指数值和兰州汉族女性峰值骨量值均较低。多元线性回归分析显示,肌肉含量、皮下脂肪、身体质量指数与骨强度指数呈正相关,内脏脂肪与骨强度指数呈反比。结论兰州市汉族成人骨强度指数和兰州市汉族女性峰值骨量均处于较低水平,且峰值骨量出现的年龄较早,50岁以后女性是骨质疏松的危险人群;肌肉量、皮下脂肪量以及BMI是骨质疏松发病的保护性因素,而内脏脂肪量是骨质疏松发病的危险因素。
Objective To investigate and analyze the characteristics of adult bone strength index and its relationship with body composition in Han nationality in Lanzhou, and provide a reference for the prevention and intervention of osteoporosis in this area. Methods Bone mineral density (BMI) and body composition were measured in 2291 adult Han Chinese male and female adults (1021 males and 1270 females) using ultrasonic bone densitometry and bioelectrical impedance analyzer. Multivariate stepwise regression analysis was used to analyze the bone strength index And body composition of the relationship between the indicators. Results The strength index of Han nationality in Lanzhou continued to decrease with age, and the peak bone mass appeared in 20 ~ 30 years old. The bone strength index decreased rapidly in men aged 20-30 years and 50-60 years, while the female’s bone strength index decreased rapidly after 50 years old. The bone strength index of Han nationality men in each age group was higher than that of females in same age group. Compared with other indexes measured by the same method in other ethnic groups and regions, the results showed that the Han nationality adult bone strength index and the Lanzhou Han female peak bone mass value were both lower. Multivariate linear regression analysis showed that muscle content, subcutaneous fat, body mass index were positively correlated with bone strength index, and visceral fat was inversely proportional to bone strength index. Conclusion The bone mineral density index of adult Han nationality in Lanzhou is lower than that of Han nationality in Lanzhou, and the age of peak bone mass appears earlier. After 50 years old, the female is the risk group of osteoporosis. The amount of muscle, the amount of subcutaneous fat And BMI is a protective factor in the pathogenesis of osteoporosis, visceral fat mass is a risk factor for the pathogenesis of osteoporosis.
