目的应用主观整体评价(subjective global assessment,SGA)对入院老年心脑血管病患者进行营养评价,分析其评估价值。方法采用前瞻性定点连续抽样的方法,选择新入院老年心脑血管患者256例作为研究对象,采用SGA进行营养评价,分析其与传统单项指标的相关性,评估SGA在老年心脑血管病患者营养评价中的价值。结果体质指数(body mass index,BMI)评价营养不良发生率为14.45%,SGA筛查显示营养不良发生率为23.05%。SGA灵敏度为70.27%,特异度为84.93%。SGA和传统营养评价方法一致性检验Kappa值=0.44,一致性较差。根据Spearman相关分析,SGA评分与传统营养评价指标BMI、白蛋白、血红蛋白、总淋巴细胞有相关性,差异具有统计学意义(P<0.05)。SGA与临床结局相关指标感染具有相关性(P<0.05),与并发症、住院时间无相关性(P>0.05)。结论 SGA在老年心脑血管病患者营养评价中具有较好的适用性,可能对老年心脑血管病患者的临床结局的预测具有参考意义。
Objective To evaluate the nutritional value of hospitalized elderly patients with cardio-cerebral vascular diseases by subjective global assessment (SGA). Methods A prospective continuous sampling method was used to select 256 newly admitted elderly patients with cardio-cerebral vascular diseases as the research object. Nutritional evaluation was performed by SGA. The correlation between SGA and traditional single-index was evaluated. The effects of SGA on nutrition of elderly patients with cardiovascular and cerebrovascular diseases Evaluation of the value. Results Body mass index (BMI) rate of malnutrition was 14.45%. SGA screening showed that the incidence of malnutrition was 23.05%. SGA sensitivity of 70.27%, specificity of 84.93%. SGA and traditional nutrition evaluation method consistency test Kappa value = 0.44, consistency is poor. According to Spearman correlation analysis, there was a significant correlation between SGA score and BMI, albumin, hemoglobin and total lymphocyte (P <0.05). There was a significant correlation between SGA and clinical outcome (P <0.05), but no correlation with SGA and hospital stay (P> 0.05). Conclusion SGA has good applicability in nutritional evaluation of elderly patients with cardiovascular and cerebrovascular diseases and may be of reference value for the prediction of clinical outcome in elderly patients with cardiovascular and cerebrovascular diseases.
