目的探讨癫患儿共患焦虑障碍的发病情况和临床特征。方法采用儿童焦虑性情绪障碍筛查量表(SCARED),对2009年12月-2010年4月在北京大学第一医院儿科就诊的165例7~16岁癫患儿进行筛查,以SCARED量表分≥23分者视为焦虑障碍。采用SPSS14.0软件对不同性别、不同年龄组的癫患儿共患焦虑障碍情况进行χ2检验,对其分值进行秩和检验。结果 1.在筛查的165例癫患儿中,焦虑障碍总体共患率为27.3%(45/165例);女童(25例,37.3%)明显高于男童(20例,20.4%),差异有统计学意义(χ2=5.734,P<0.05)。7~12岁学龄儿童共患焦虑障碍38/138例(27.5%),13~16岁青春期儿童7/27例(25.9%),2个年龄组比较差异无统计学意义(χ2=0.000,P>0.05)。2.对不同性别癫患儿SCARED筛查结果进行比较,在各项目分、5个因子分及总分几个方面,女童得分均高于男童,其中7个项目分(第3、4、10、26、32、35、39小项目),2个因子分(广泛性焦虑、社交恐怖)及总分存在统计学差异(Pa<0.05)。3.对7~12岁、13~16岁2个年龄组癫患儿进行比较,在总分上差异无统计学意义,但在个别小项目上,如量表中第13条(我总担心让我一个人睡觉),7~12岁年龄组癫患儿分值明显高于13~16岁年龄组的癫患儿,差异有统计学意义(P<0.01)。结论癫患儿共患焦虑障碍的发生率较高,女童更易发生焦虑障碍,应加强对癫患儿精神心理状况的关注。
Objective To investigate the incidence and clinical features of comorbid anxiety disorders in children with epilepsy. Methods A total of 165 children aged 7-16 years old with epilepsy were enrolled in the Pediatric Anxiety Disorder Scale (SCARED) from December 2009 to April 2010 in Peking University First Hospital. SCARED Score points ≥ 23 points were considered as anxiety disorders. SPSS14.0 software was used to test the anxiety disorders of children with epilepsy in different genders and different age groups byχ2 test and rank sum test. Results Among the 165 children with epilepsy, the overall prevalence rate of anxiety disorders was 27.3% (45/165), while that of girls (25.37.3%) was significantly higher than that of boys (20.4%) , The difference was statistically significant (χ2 = 5.734, P <0.05). There were 38/138 cases (27.5%) of children aged 7-12 years with comorbid anxiety disorder, 7/27 cases (13.2%) of adolescent children aged 13-16 years, there was no significant difference between the two age groups (χ2 = 0.000, P > 0.05). SCARED screening results of children with different genders of epilepsy were compared in terms of the project points, five factors and scores in several areas, the girls scored higher than boys, of which seven projects (3,4 , 10,26,32,35,39 small items), two factors (generalized anxiety, social terror) and the total score there is a statistically significant difference (Pa <0.05). There was no significant difference in the total score between children aged 7 ~ 12 years and 13 ~ 16 years old with epilepsy. However, on some small items, Worried about letting me sleep alone). The score of epilepsy in 7 ~ 12 age group was significantly higher than that of epilepsy in 13 ~ 16 age group, the difference was statistically significant (P <0.01). Conclusions The incidence of anxiety disorders in children with epilepsy is high, and girls are more likely to have anxiety disorders. Therefore, attention should be paid to the mental and psychological status of children with epilepsy.
