[目的]探讨用腮腺功能动态检测来预测鼻咽癌患者放射敏感性的可行性。[方法]用腮腺ECT前瞻性研究42例鼻咽癌病人放疗前、放疗20Gy及60Gy时腮腺摄取功能、排泌功能,统计分析皮肤黏膜敏感组与不敏感组间、鼻咽肿瘤组织局控组与残留组间,左、右腮腺摄取功能减退率、排泌功能减退率等指标间的差异。[结果]在放射敏感组,左右腮腺排泌功能减退率在20Gy、60Gy时均较不敏感组更明显(P<0.05),而两组间摄取功能减退率无差异。左右腮腺排泌功能减退在60Gy时局控组较残留组更明显,但20Gy时两组间左右腮腺排泌功能减退率无差异。左右腮腺摄取功能减退率在放疗20Gy至60Gy时变化不明显。左右腮腺排泌功能减退极为明显,20Gy时左、右腮腺极度减退者分别为23例(54.76%)、19例(45.24%),60Gy时左、右腮腺极度减退者分别为33例(78.57%)、35例(83.33%)。[结论]腮腺ECT动态检测结果显示,射线主要损伤腮腺的排泌功能,在皮肤黏膜放射反应敏感组,20Gy、60Gy时左右腮腺排泌功能减退率明显增加。在局控组,60Gy时左右腮腺排泌功能减退较残留组更明显,有可能作为预测鼻咽癌放射敏感性的指标。
[Objective] To explore the feasibility of using parotid gland dynamic test to predict the radiosensitivity of nasopharyngeal carcinoma patients. [Methods] The parotid gland ECT prospective study of 42 patients with nasopharyngeal carcinoma before radiotherapy, radiotherapy 20Gy and parotid gland 60Gy uptake and excretion function, statistical analysis of skin and mucous membrane sensitive group and not sensitive group nasopharyngeal tumor control group And the residual group, left and right parotid glands function decline rate, excretion function decline rate and other indicators between the differences. [Results] In the radiosensitive group, the excretion rate of left and right parotid glands was significantly lower than that of the non-sensitive group at 20 Gy and 60 Gy (P <0.05), while there was no difference between the two groups. Left and right parotid excretion dysfunction in the control group 60Gy control group more obvious than the residual group, but 20Gy left parotid between the two groups no difference in excretion function. Left and right parotid uptake rate of decline in the 20Gy 60Gy radiotherapy did not change significantly. There were 23 cases (54.76%) and 19 cases (45.24%) with paroxysmal hypo parotid hyperplasia at 20Gy respectively, and 33 cases (78.57% ), 35 cases (83.33%). [Conclusion] The results of dynamic examination of parotid ECT showed that the excretion function of the parotid gland, which is mainly damaged by radiation, was significantly increased in the mucosal radiosensitive sensitive group at 20Gy and 60Gy. In the local control group, excretion of parotid glands at 60 Gy was more pronounced than that of the residual group, which may be used as an index to predict radiosensitivity of NPC.
