目的比较老年高血压病合并糖尿病患者的血管情况、静脉穿刺效果及不良反应。方法 1将安徽省立医院干部四病区≥80岁高血压病患者180例分为两组:高血压病组及高血压病合并糖尿病组,通过浅静脉血管评估标准对血管进行评估比较;2比较两组患者一次性穿刺成功率、伤口肿胀、肢体淤血、针头移位和输液渗漏、静脉炎等指标,并通过问卷调查对护理满意率进行评价;3将不同病程的高血压病合并糖尿病患者依据病程<20年及≥20年分组,比较两组血管评估及穿刺并发症、护理满意率等指标。结果1高血压病组的血管评估优于合并糖尿病组(P=0.047)。2高血压病组的一次性穿刺成功率(P=0.032)、伤口肿胀(P=0.035)、肢体淤血(P=0.021)、针头移位和输液渗漏(P=0.018)、静脉炎(P=0.048)、护理满意率(P=0.027)均优于合并糖尿病组。3糖尿病病程<20年组的血管评估、穿刺并发症、护理满意率等指标与≥20年组比较,差异无统计学意义(P>0.05)。结论高血压病患者血管评估情况、并发症、护理满意率等指标明显优于高血压病合并糖尿病患者。
Objective To compare the vascular status, venipuncture effect and adverse reactions in elderly hypertensive patients with diabetes mellitus. Method 1 Divide 180 cases of hypertensive patients ≥80 years old in four wards of cadres of Anhui Provincial Hospital into two groups: hypertension group and hypertension with diabetes mellitus group. The success rate of one-time puncture, wound swelling, limb congestion, needle displacement and infusion leakage, phlebitis and other indicators were compared between the two groups, and the satisfaction rate of nursing was evaluated through questionnaire. 3 Different stages of hypertension with diabetes mellitus The patients were grouped according to the course of disease <20 years and ≥20 years. The vascular assessment and puncture complication and nursing satisfaction rate were compared between the two groups. Results 1 The vascular assessment of hypertensive patients was better than that of patients with diabetes mellitus (P = 0.047). The success rate of one-time puncture (P = 0.032), wound swelling (P = 0.035), limb congestion (P = 0.021), needle displacement and infusion leakage (P = 0.018) and phlebitis (P = 0.048), nursing satisfaction rate (P = 0.027) were better than those with diabetes mellitus. There was no significant difference in blood vessel evaluation, puncture complication, nursing satisfaction rate and other indexes between 20 years and 20 years of course of diabetes mellitus (P> 0.05). Conclusion Hypertension patients with vascular assessment, complications, nursing satisfaction rate and other indicators were significantly better than those with hypertension and diabetes.
