目的探讨伴发相应部位浸润性癌的乳腺外派杰氏病(EMPD)的临床病理学特征、组织学发生及特点。方法对10例伴发相应部位浸润性癌的EMPD进行临床病理学分析,并行AB-PAS染色和CEA、CK7、CK20、GCDFP-15、HMB45免疫组化标记。结果10例CEA强阳性,AB-PAS呈不同程度阳性,CK7阳性7例,其中6例为伴发皮肤浸润性腺癌,CK20阳性3例,其中2例为伴发直肠腺癌,GCDFP-15阳性6例为伴发皮肤浸润性腺癌,HMB45均阴性。结论在伴发相应部位浸润性癌的EMPD中可以分为2种:(1)伴发皮肤浸润性腺癌;(2)伴发邻近内脏器官癌肿。免疫组化CK7、CK20、GCDFP-15有助于区分,在CK7、GCDFP-15阴性,CK20阳性的EMPD中应警惕伴发邻近内脏器官癌肿的可能。
Objective To investigate the clinicopathological features, histological features and histopathological features of Ectopic mays disease (EMPD) associated with invasive carcinoma of the corresponding sites. Methods EMPD in 10 cases with invasive carcinoma of the corresponding site was analyzed by clinicopathological analysis. The expression of CEA, CK7, CK20, GCDFP-15 and HMB45 were detected by AB-PAS staining. Results 10 cases of CEA were strongly positive, AB-PAS was positive to some extent, CK7 was positive in 7 cases, of which 6 cases were invasive adenocarcinoma of the skin. CK20 was positive in 3 cases, of which 2 cases were associated with rectal adenocarcinoma, GCDFP-15 positive 6 cases of invasive adenocarcinoma of the skin, HMB45 were negative. Conclusion There are two types of EMPD associated with invasive carcinoma at the corresponding sites: (1) invasive adenocarcinoma of the skin; and (2) cancer associated with adjacent organs. Immunohistochemistry CK7, CK20, GCDFP-15 help distinguish, in CK7, GCDFP-15 negative, CK20-positive EMPD should be vigilant associated with the possibility of adjacent organs and cancer.
