目的:通过前瞻性临床试验评估上颌窦内嵌骨块技术在余留牙槽嵴高度不足5mm时的疗效。方法:采用上颌窦内嵌骨块技术,借助环形钻对32例50颗上颌后牙行经上颌窦提升术,并同期植入短种植体。缺牙区平均余留牙槽嵴高度为3.34mm(0.96mm~4.96mm)。术后6个月行暂冠修复,进行咬合训练及软组织塑形,3个月后永久修复。定期复诊,检查上颌窦及种植体骨结合情况,CBCT图像测量骨增量水平。结果:所有病例均无种植体松动、脱落及上颌窦炎发生,CBCT示种植体与周围组织形成良好的骨性结合。经平均16个月的随访观察,临床效果良好。术后平均窦底垂直骨增量为5.38mm。讨论:1上颌窦内嵌骨块技术的优点和解剖生理基础;2手术注意事项;3短种植体的临床应用。结论:该上颌窦内嵌骨块技术联合短种植体同期植入是一种创伤小、操作简单、行之有效的上颌后牙区种植外科技术,可用于上颌窦底垂直骨量严重不足,余留牙槽骨高度不足5mm的病例。
OBJECTIVE: To evaluate the efficacy of maxillary sinus embedded bone technique in prospective clinical trials when residual alveolar ridge height is less than 5 mm. Methods: Using maxillary sinus embedded bone technique, 32 cases of 50 maxillary posterior teeth were performed maxillary sinus lift with ring drill, and a short implant was implanted in the same period. Edentulous area the average remaining alveolar ridge height 3.34mm (0.96mm ~ 4.96mm). 6 months after the line crown repair, bite training and soft tissue shaping, 3 months after permanent repair. Regular follow-up, check the maxillary sinus and implant bone union, CBCT image measurement of bone increments. Results: There was no loosening, exfoliation and maxillary sinusitis in all the cases. CBCT showed that the implant formed good osseous union with the surrounding tissues. After an average of 16 months of follow-up observation, the clinical effect is good. The average postoperative average sinus bone augmentation was 5.38mm.
