绝经后妇女因为卵巢功能衰退,雌激素水平低,生殖器官萎缩,特别是宫颈萎缩,穹窿消失,给取出宫内节育器(IUD)造成一定困难。现将本院6年来在取IUD前应用不同药物的临床效果比较介绍如下。 一、资料方法 1.观察对象 1995～2000年绝经后来本院自愿取器者116例,外院取器失败44例,共160例。平均年龄56(45～62)岁,放置IUD时间22(10～26)年,绝经时间12(0.5～22)年。有下腹坠胀不适,腰骶部疼痛,阴道少量血性分泌物23例。均无药物过敏史及手术禁忌证。随机分为4组,各组40例。 2.方法 A组取器前半小时服用沙丁胺醇4.8mg;B组取器前3h阴道置入米索前列醇0.4mg;C组取器前1周顿服尼尔雌醇4mg;D组取器前两周口服尼尔雌醇1mg/3日,连服5mg。均按节育手术操作常规取出宫内节育器手术。 3.宫颈软化标准 ①软化:宫口略扩张,用3号扩宫棒无
