【目的】观察叶酸及维生素B12在强化促红细胞生成素(erythropoietin,EPO)在早产儿贫血中的防治作用。【方法】将2006年1月—2010年5月间在本院新生儿科住院的早产儿69例,首先按出生体重分为两组;1 000 g≤出生体重<1 500 g的极低出生体重儿48例为第一组,再随机分为治疗组Ⅰ26例,对照组Ⅰ22例;出生体重<1 000 g的超低出生体重儿21例为第二组,并随机分为治疗组Ⅱ11例,对照组Ⅱ10例,两组均于生后第7天给予EPO 250 U/(kg.d),3次/周,共6周,同时加服铁剂[元素铁3~5 mg/(kg.d)]。治疗组在此基础上加服叶酸[200μg/(kg.d)],以及维生素B12[30μg/(kg.周)]。对照组未予强化叶酸及维生素B12。【结果】6周后治疗组外周血血红蛋白,红细胞计数,红细胞压积均显著高于对照组,红细胞平均体积显著低于对照组。且超低出生体重儿组的疗效较极低出生体重儿组更显著。【结论】叶酸及维生素B12可加强EPO防治早产儿贫血的疗效,且出生体重愈低,疗效愈显著。
【Objective】 To observe the preventive and therapeutic effects of folic acid and vitamin B12 in the treatment of anemia in premature infants with erythropoietin (EPO). 【Methods】 Sixty-nine preterm infants hospitalized in neonatal department of our hospital from January 2006 to May 2010 were divided into two groups according to their birth weight: 1 000 g ≤ very low birth weight (birth weight <1500 g) 48 cases were divided into the first group and then randomly divided into treatment group I26 cases, control group I22 cases; birth weight <1000 g of low birth weight infants 21 cases were the second group, and were randomly divided into treatment group II11 cases, The control group, n = 10, were given EPO 250 U / (kg · d) on the 7th day after birth for 3 weeks / week for 6 weeks. The effect of ultra-low birth weight children group was more significant than that of low birth weight children group. 【Conclusion】 Folic acid and vitamin B12 can enhance the efficacy of EPO in prevention and treatment of anemia in premature infants. The lower the birth weight, the more obvious the curative effect.
