目的观察痛风合并糖代谢异常患者临床相关指标。方法收集痛风患者139例,根据糖代谢情况分为痛风合并糖耐量正常组(A组)44例、痛风合并糖耐量受损组(B组)49例、痛风合并糖尿病组(C组)46例,计算三组体质量指数(BMI),记录收缩压(SBP)、舒张压(DBP);采用ELISA法检测空腹血糖(FPG)、餐后0.5 h血糖(0.5 h PG)、餐后2 h血糖(2 h PG)、血脂、糖化血红蛋白(Hb A1c),及胰岛素分泌指标空腹胰岛素(Fins)、餐后30 min胰岛素(Ins30 min)、餐后2 h胰岛素(Ins120min);统计并发症情况。结果 A、B、C组Hb A1c、FPG、0.5h PG、2h PG、BMI、TG、TC、LDL-C均逐渐升高,Ins30 min逐渐下降,组间比较均P<0.05;FIns、Ins120 min、SBP、DBP组间比较差异无统计学意义(均P>0.05)。A组伴有并发症8例(18.18%),B+C组22例(23.16%),B+C组并发症发生率高于A组(χ2=5.1492、P<0.05)。结论痛风合并糖代谢异常患者Hb A1c、FPG、0.5 h PG、2 h PG、Ins30 min水平升高,发生糖尿病、高血脂机会更大,但对血压及Fins水平影响不大。
Objective To observe the clinically relevant indexes of gout patients with abnormal glucose metabolism. Methods Ninety-nine patients with gout were divided into four groups: gout group (group A), gout group (49 cases), gout group (group B), gout group with diabetes mellitus (group C) The body mass index (BMI), SBP and DBP were recorded. The levels of fasting blood glucose (FPG), postprandial 0.5 h blood glucose (0.5 h PG), postprandial blood glucose 2 h (2 h PG), blood lipid, Hb A1c and insulin fasting insulin (Fins), 30 min insulin (Ins30 min), and 2 h insulin (Ins120 min) were measured. Results The levels of Hb A1c, FPG, 0.5h PG, 2h PG, BMI, TG, TC and LDL-C in groups A, B and C increased gradually, , SBP, DBP were no significant difference between groups (all P> 0.05). There were 8 cases (18.18%) with complications in group A, 22 cases (23.16%) in group B + C, and the incidence of complications in group B + C was higher than that in group A (χ2 = 5.1492, P <0.05). Conclusions HbA1c, FPG, 0.5 h PG, 2 h PG, Ins30 min in patients with gout complicated with abnormal glucose metabolism have higher level of diabetes, hyperlipidemia, but little effect on blood pressure and Fins level.
