目的:探讨高压氧(HBO)辅助治疗对脑肿瘤术后放疗患者生命质量及免疫功能指标的影响。方法:选取2015年12月至2017年12月山西省肿瘤医院神经外科收治的脑部恶性肿瘤术后放疗患者100例作为研究对象,按照数字表法随机分为观察组和对照组,每组50例。对照组患者在术后2~4周开始接受常规放疗,观察组患者在对照组治疗的基础上予以HBO治疗(HBO治疗结束30 min后开始接受放疗)。记录治疗过程中放射性脑损伤并发症发生情况。在治疗前后分别采集患者外周静脉血并检测外周血CD3n +、CD4n +、CD8n +水平。治疗结束后所有患者均随访1年,记录治疗后1年生存率,并采用SF-36健康调查量表评估其生命质量。n 结果:观察组患者治疗后放射性脑损伤并发症发生率(14.0%)显著低于对照组(50.0%),差异有统计学意义(n P<0.01);观察组患者外周血CD4n +细胞百分比、CD4n +/CD8n +值均明显高于治疗前,同时也显著高于对照组治疗后(n P<0.05)。治疗后1年,观察组患者生存率(84.0%,42/50)明显高于对照组(66.0%,33/50)(n P<0.05);观察组患者在生理功能、生理职能、总体健康、社会功能、精神健康维度的得分均显著高于对照组(n P<0.05)。n 结论:HBO治疗可提高脑肿瘤术后放疗患者的免疫功能,改善其生命质量。“,”Objective:To investigate the effects of hyperbaric oxygen (HBO) on the quality of life and immune function in patients with brain tumors undergoing postoperative radiotherapy.Methods:A total of 100 patients with brain tumors admitted in Shanxi Provincial Cancer Hospital from December 2015 to December 2017 were selected as the research objects and divided into observation group and control group by the random number table method, with 50 cases in each group. Patients in both groups received conventional radiotherapy 2-4 weeks after operation. Patients in the observation group received HBO 30 minutes before the same treatment of the control group. The incidence of radiation-induced brain injury complications during treatment was recorded. Patients′ peripheral venous blood was collected before and after treatment, and the peripheral blood CD3n + , CD4n + , and CD8n + levels were measured. Both groups were followed up for one year after treatment, and the one-year survival rate was recorded. The 36-Item Short Form Health Survey (SF-36) was used to evaluate the patients′ quality of life one year after treatment.n Results:After treatment, the incidence of radiation-induced brain injury complications in the observation group (14%) was significantly lower than that in the control group (50%), with statistical difference (n P<0.01). After treatment, the percentage of peripheral blood CD4n + cells and the ratio of CD4n + / CD8n + in the observation group were significantly increased (n P<0.01), and were significantly higher than those in the control group after treatment (n P<0.01). The one-year survival rate in the observation group after treatment (84.0%, 42/50) was significantly higher than that in the control group (66.0%, 33/50) (n P<0.05). Compared with the control group, the patients in the observation group had significantly higher scores in physical functioning, physical role functioning, general health perceptions, social role functioning, and mental health one year after treatment (n P<0.05).n Conclusion:HBO therapy can improve the immune function and quality of life of brain tumor patients undergoing postoperative radiotherapy.
