目的观察双氯芬酸钠栓Ⅱ和宫颈注射利多卡因、阿托品用于负压吸宫术中镇痛的效果。方法将在我院住院部要求终止妊娠的早孕孕妇因以及因药流后胎物残留需行负压吸宫(清宫术)200例随机分成实验组与对照组,每组各100例。实验组术前1.5小时以1枚双氯芬酸钠Ⅱ塞肛后进行手术,术前向宫颈注射利多卡因和阿托品的混合液。观察孕妇术中术后的反应;对照组术前术中不用任何药物,按常规负主吸宫术的常规手术操作进行。结果实验组孕妇疼痛明显轻于对照组(P<0.05),无1例人工流产综合征发生,术中出血亦较少,术前术后心率变化无统计学意义;而对照组手术前后心率比较差异显著(P<0.05),两组孕妇手术前后血压变化无统计学意义(P>0.05)。结论双氯芬酸钠(Ⅱ)和宫颈注射利多卡因、阿托品用于负压吸宫术操作简单、经济、安全、且能有效地减轻负压吸宫术时患者的痛苦,预防人流综合佂的发生,可以在基层卫生院及门诊无麻醉师的情况下开展,以减轻患者的痛苦。
Objective To observe the effect of diclofenac sodium suppository Ⅱ and cervical injection of lidocaine and atropine for analgesia in vacuum aspiration. Methods The pregnant women who requested termination of pregnancy in our hospital were divided into experimental group and control group randomly, 200 cases each with 100 cases in each group. 1.5 hours before surgery in the experimental group with 1 diclofenac sodium Ⅱ anus after surgery, preoperative injection of lidocaine and atropine mixed solution. Observed the postoperative reaction of pregnant women; control group without any preoperative surgery, according to the conventional negative suction sphincter of conventional surgical procedures. Results The pregnant women in the experimental group were significantly less painful than those in the control group (P <0.05), and no one had an induced abortion syndrome with less bleeding during operation and no significant changes in heart rate before and after operation. The heart rate of the control group before and after surgery was significantly lower than that of the control group (P <0.05). There was no significant difference in blood pressure between the two groups before and after operation (P> 0.05). Conclusion Diclofenac sodium (Ⅱ) and lidocaine and atropine in cervix are simple, economical and safe for vacuum suction aspiration, which can effectively alleviate the pain of patients undergoing vacuum suction aspiration and prevent the occurrence of general flow. It can be carried out without the aid of anesthetists in primary health care clinics and out-patient clinics to alleviate the suffering of patients.
