目的进一步评价无创正压通气在急性左心衰合并低氧血症抢救中的疗效。方法对47例急性左心衰合并低氧血症患者随机分组。治疗组25例采用常规药物治疗加BiPAP呼吸机面罩或鼻罩双水平正压通气,对照组22例采用常规药物治疗加鼻导管吸氧。结果治疗组经无创正压通气后23例患者在30min~2h症状好转,呼吸减慢(P<0.05)、心率下降(P<0.05)、PaO2上升(P<0.05)、肺底部湿啰音减少或消失(P<0.05),总有效率92%。对照组在相应时间内仅13例好转,有效率59.1%。两组相比差异有统计学意义(P<0.05)。结论无创正压通气治疗急性左心衰合并低氧血症效果显著。
Objective To further evaluate the efficacy of non-invasive positive pressure ventilation in the rescue of acute left ventricular failure with hypoxemia. Methods 47 cases of acute left ventricular failure with hypoxemia were randomly divided into groups. Twenty-five patients in the treatment group received bi-level positive pressure ventilation with BiPAP respirator mask or nasal mask, while those in the control group received conventional drug therapy and nasal catheter oxygenation. Results In the treatment group, the symptoms of the patients in the treatment group were improved after 30min to 2h, the breathing rate was decreased (P <0.05), the heart rate was decreased (P <0.05), the PaO2 was increased (P <0.05) Or disappeared (P <0.05), the total effective rate was 92%. In the control group only 13 cases improved in the corresponding time, the effective rate was 59.1%. The difference between the two groups was statistically significant (P <0.05). Conclusion Noninvasive positive pressure ventilation is effective in treating acute left ventricular failure with hypoxemia.
