830 nm近红外光联合308 nm单频准分子光治疗白癜风的临床疗效观察

目的:探讨830 nm近红外光(IR-A)联合308 nm单频准分子光(MEL)照射对白癜风的治疗效果.方法:选择皮肤科门诊白癜风患者123例,每例患者色素减退面积占全身皮肤总面积≤10%,随机分成3组,分别给予IR-A、MEL及二者联合治疗,每周2次,连续进行12周并作出疗效评价.结果:IR-A与MEL联合治疗组40例,显效率72.5%,高于IR-A组(40例,显效率25.0%)与MEI,组(43例,其中3例中途放弃治疗,显效率47.5%).差异均有统计学意义(P均<0.05).联合治疗组起效的速度较其他2组更快.不良反应为红斑、轻度瘙痒等,其中IR-A组的不良反应最为轻微,可以忽略;MEL组不良反应最重,且消退最慢;联合治疗组不良反应较轻,能很快自行消退.结论:IR-A与MEL联合应用可以提高治疗白癜风效果,起效速度更快,并减轻不良反应.“,”Objective:To investigate the clinical efficacy of 830 mn near infrared ray(IR-A) radiation combined with 308 nm monochromatic excimer light(MEL) for vitiligo.Methods:A total of 123 vitiligo paticnts with depigmented area≤10% of the total skin area were randomly divided into 3 groups,i.e,IR-A,MEL,and combination of IR-A and MEL treatment.Patients were treated twice weekly for 12 weeks.Result:Treatments with combination of IR-A and MEL yielded significantly higher effective rates than that with either IR-A or MEL alone (effective rates:72.5% for combination of IR-A and MEL;25.0% for IR-A;47.5% for MEL).In addition,repigmentation appeared earlier in patients treated with combination of IR-A and MEL than in patients treated with either IR-A or MEL alone.Adverse reactions included etythema,mild pruritus and others.IR-A-induced adverse reactions were negligible,while MEL induced severe side effects,which disappeared slowly.In contrast,combination of IR-A and MEL caused mild side effects,which disappeared rapidly and spontaneously.Conclusion:Combination of IR-A and MEL(UVB) can improve the therapeutic efficacy for vitiligo,rapidly induce repigmentation,and reduce the severity of adverse effects.
