目的观察经皮经肝组织粘合剂(TH 胶)栓塞治疗食管静脉曲张的远期效果。方法经皮经肝 TH 胶栓塞治疗食管静脉曲张47例,术后定期复查胃镜及 CT,了解食管下段曲张静脉内 TH胶转归和静脉曲张消失情况。结果 45例食管静脉曲张患者术后显效率100%,随访6~52个月,平均(31±22)个月,静脉曲张复发10例;随访过程中出血4例,再出血率8.9%。胃镜及 CT 随访显示,食管黏膜下曲张静脉内 TH 胶逐渐脱失,而食管旁和胃底贲门周围静脉内 TH 胶长期滞留栓塞。结论经皮经肝栓塞术能使 TH 胶到达食管下段的黏膜下曲张静脉,使曲张静脉长期闭塞,能有效的治疗食管静脉曲张,预防复发。
Objective To observe the long-term effect of percutaneous transhepatic embolization (TH glue) embolization on esophageal varices. Methods Forty-seven patients with esophageal varices were treated with percutaneous transhepatic TH glue plug. Gastroscopy and CT were performed regularly to find out the disappearance of TH glue and disappearance of varicose veins in the lower esophageal varices. Results In 45 cases of esophageal varices, the effective rate was 100%. The patients were followed up for 6 to 52 months with an average of (31 ± 22) months and 10 cases of recurrent varicosis. Bleeding was followed up in 4 cases and the rate of rebleeding was 8.9%. Gastroscopy and CT follow-up showed that esophageal submucosal thyroid gradually disappear TH glue, and near the esophageal and gastric cardia around the long-term retention of TH glue plastic embolization. Conclusion Percutaneous transhepatic embolization can cause TH glue to reach the submucosal varicose veins in the lower esophagus and cause long-term varicose vein occlusion, which can effectively treat esophageal varices and prevent recurrence.
