目的研究妊娠期糖尿病孕妇孕晚期血脂水平的变化及其与新生儿出生体重的相关性。方法选择正常糖耐量孕妇(对照组)和妊娠期糖尿病孕妇(妊娠期糖尿病组)各52例,采用日立7080型全自动生化分析仪测定血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)和脂蛋白(a)[LP(a)],同时记录两组孕妇的年龄、孕周、妊娠前体重与新生儿出生体重等。结果 GDM组TG、LDL-C均显著高于对照组,差异有统计学意义(P<0.05)。TG与新生儿出生体重呈显著正相关(P<0.05),LP(a)与新生儿出生体重呈显著负相关(P<0.05)。结论妊娠期糖尿病孕妇存在以TG、LDL-C升高为特点的高脂血症,与正常糖耐量孕妇相比,伴发了更为严重的血脂代谢异常,并明显影响着新生儿的出生体重,因此对妊娠期糖尿病孕妇进行孕晚期血脂水平的监测和必要的饮食指导具有重要的临床意义。
Objective To study the changes of serum lipids in pregnant women with gestational diabetes mellitus during the third trimester of pregnancy and its relationship with birth weight. Methods 52 cases of normal glucose tolerance pregnant women (control group) and gestational diabetes mellitus (gestational diabetes mellitus group) were selected. Hitachi 7080 automatic biochemistry analyzer was used to measure serum total cholesterol (TC), triglyceride (TG) (HDL-C), low density lipoprotein cholesterol (LDL-C) and lipoprotein (a) [LP (a)]. The age, gestational age, Birth weight and so on. Results TG and LDL-C in GDM group were significantly higher than those in control group (P <0.05). There was a significant positive correlation between TG and newborn birth weight (P <0.05). There was a significant negative correlation between LP (a) and newborn birth weight (P <0.05). Conclusions Pregnant women with gestational diabetes mellitus have hyperlipidemia characterized by elevated TG and LDL-C. Compared with normal glucose tolerance pregnant women, they are associated with more serious dyslipidemia and significantly affect the birth weight of newborns , So pregnant women with gestational diabetes for the third trimester of blood lipid levels monitoring and necessary diet guidance has important clinical significance.
