目的探讨经阴道超声检查对宫角妊娠的诊断价值。方法选择我院2010年9月~2012年9月83例疑似宫角妊娠患者,采用自体对照研究,先给予腹部超声检查后,再行经阴道超声检查,将检查结果与病理结果比较,对检查准确率进行比较。结果 83例患者中有80例经病理检查证实为宫角妊娠。经腹超声检查宫角妊娠46例,误诊37例,其中31例误诊为输卵管妊娠,6例未准确定位;经阴道超声检查宫角妊娠70例,符合率为87.5%;误诊13例,其中10例误诊为输卵管妊娠,3例未准确定位;经阴道超声检查准确率明显高于经腹超声,两组检查方法准确率差异具有显著性,有统计学意义(P<0.05)。经阴道超声显示宫角妊娠分三种类型,分别为胚囊型26例,占37.14%;破裂型20例,占28.57%;包块型24例,占34.29%。结论宫角妊娠采用经腹超声误诊率较高,经阴道超声具有探头接近靶器官,超声探头频率高,显示清晰的特点,明显提高了诊断的准确率,具有较高的诊断价值。
Objective To investigate the value of transvaginal ultrasound in the diagnosis of uterine horn pregnancy. Methods Eighty-three patients with suspected uterine horn pregnancy in our hospital from September 2010 to September 2012 were enrolled in this study. Autologous control study was conducted. Abdominal ultrasonography was performed before transvaginal ultrasonography was performed. The results were compared with the pathological findings. Rate for comparison. Results Eighty-three of 83 patients were proved to have uterine horn pregnancy by pathological examination. Transvaginal ultrasound examination of 46 cases of uterine horn pregnancy, misdiagnosis in 37 cases, of which 31 cases were misdiagnosed as tubal pregnancy, 6 cases were not accurately positioned; Transvaginal ultrasound in 70 cases of uterine horn pregnancy, the coincidence rate was 87.5%; misdiagnosed in 13 cases, of which 10 Cases misdiagnosed as tubal pregnancy, 3 cases were not accurately located; transvaginal ultrasound was significantly higher than the accuracy of transabdominal sonography, the accuracy of the two groups was significantly different, with statistical significance (P <0.05). Transvaginal ultrasound showed that there were three types of cornual pregnancy, which were 26 cases of embryo sac, accounting for 37.14%, 20 cases of rupture, accounting for 28.57%, and 24 cases (34.29%). Conclusions The incidence of misdiagnosis of uterine hormones in pregnant women with high uterine horn pregnancy is high. Transvaginal sonography has the advantages of close proximity to the target organ, high frequency of the ultrasonic probe, and clear features. The accuracy of the diagnosis is significantly improved, which is of high diagnostic value.
