目的 研究糖尿病患者白蛋白尿排泄率 ( AER)与骨代谢变化的关系。方法 测定 132例 型糖尿病患者和 2 5例对照组血钙、磷、碱性磷酸酶、尿钙 /肌酐值。糖尿病患者根据 AER分成正常白蛋白尿组 ( AER<2 0 μg/ min)、微量白蛋白尿组 ( 2 0 μg/ min≤AER<2 0 0 μg/ min)及大量白蛋白尿组 ( AER≥ 2 0 0 μg/ min)。并分别测定腰椎 2~ 4、髋部骨密度。结果 糖尿病患者与对照组相比 ,尿钙 /肌酐明显增高 ( P<0 .0 1) ,腰椎 2~ 4、髋部骨密度显著降低 ( P<0 .0 1)。糖尿病患者中 ,大量白蛋白尿组骨密度明显低于正常尿白蛋白组及微量尿白蛋白组 ( P<0 .0 5)。结论 糖尿病患者骨密度比正常人低 ,其中大量白蛋白尿的糖尿病患者其骨密度下降比正常白蛋白尿及微量白蛋白尿者显著
Objective To study the relationship between albuminuria excretion rate (AER) and bone metabolism in diabetic patients. Methods The levels of serum calcium, phosphorus, alkaline phosphatase and urinary calcium / creatinine were measured in 132 patients with type 2 diabetes and 25 controls. Diabetic patients were divided into normal albuminuria group (AER <20 μg / min), microalbuminuria group (20 μg / min≤AER <200 μg / min) and large albuminuria group (AER> 200 μg / min). And lumbar spine were measured 2 to 4, hip bone mineral density. Results Compared with the control group, urinary calcium / creatinine was significantly increased (P <0.01) in diabetic patients and 2-4 weeks in lumbar spine. The BMD of hip was significantly decreased (P <0.01). Diabetic patients, a large number of albuminuria group, bone mineral density was significantly lower than the normal urinary albumin group and microalbuminuria group (P <0.05). Conclusion The BMD of diabetic patients is lower than that of normal people, and the BMD of diabetics with large amount of albuminuria is lower than that of normal albuminuria and microalbuminuria
