目的研究人矮小同源盒(SHOX2)基因甲基化与血栓事的关系;了解MSP-DHPLC检测外周血DNA SHOX2基因甲基化对肺癌的诊断效能。方法用MSP-DHPLC方法检测38例肺癌患者和10例良性疾病(对照)外周血SHOX2甲基化,多谱勒超声检查肺癌患者血管情况,常规检测凝血指标。结果 38例肺癌患者中,SHOX2甲基化阳性18例,阴性20例,对照组10例中甲基化阳性2例,阴性8例,敏感性为47.4%(18/38),特异性为80.0%(8/10)。18例SHOX2甲基化阳性肺癌患者中,有4发生深静脉血栓,20例甲基化阴性患者均无血栓事件(Fisher’s exact test P=0.0415)。SHOX2甲基化阳性与阴性肺癌患者的PT、INR、a PTT、TT、纤维蛋白原、AT、D二聚体和FDP均无明显差别(P>0.05)。结论 SHOX2甲基化阳性肺癌患者具有较高的血栓事件,阳性与阴性患者凝血指标无明显差异。血浆SHOX2甲基化对肺癌有辅助诊断价值。
Objective To investigate the relationship between methylation of SHOX2 gene and thrombosis and to investigate the diagnostic efficacy of MSP-DHPLC in the detection of SHOX2 gene methylation in peripheral blood. Methods Thirty-eight cases of lung cancer and 10 cases of benign disease (control) were detected by SHOX2 methylation and Doppler ultrasonography in patients with lung cancer by MSP-DHPLC. The coagulation indexes were detected routinely. Results Of the 38 patients with lung cancer, 18 cases were positive for methylation of SHOX2 and 20 cases were negative. In the control group, 2 cases were methylated, 8 cases were negative, the sensitivity was 47.4% (18/38) and the specificity was 80.0 % (8/10). Among 18 patients with SHOX2 methylation-positive lung cancer, 4 developed deep vein thrombosis and none of the 20 methylated patients had thrombotic events (Fisher’s exact test P = 0.0415). PT, INR, PTT, TT, fibrinogen, AT, D dimer and FDP in patients with SHOX2 methylation positive and negative lung cancer showed no significant difference (P> 0.05). Conclusions Patients with SHOX2 methylation-positive lung cancer have higher thromboembolic events and there is no significant difference in coagulation index between positive and negative patients. Plasma SHOX2 methylation has diagnostic value for lung cancer.
