目的 探讨血液灌流治疗重症手足口病患儿的临床效果.方法 选择2009年9月-2015年9月就诊的重症手足口病患儿82例,按照是否接受血液灌流治疗分为血液灌流组与对照组,两组均41例.对两组患儿治疗前后肺功能、血气水平以及炎症因子表达水平进行比较与分析.结果 治疗后,两组患儿的用力肺活量(FVC)和一秒率(FEV1/FVC)值都比治疗前上升(P<0.05).治疗后血液灌流组的FVC和FEV1/FVC值都明显高于对照组(P<0.05).治疗后两组的心率和呼吸频率较治疗前明显下降(P<0.05),且血液灌流组的心率和呼吸频率下降程度较对照组更显著(P<0.05).血液灌流组治疗后的血清IL-6和C-反应蛋白(CRP)值分别为(58.26±11.84) pg/mL和(1.52±0.65) mg/L,而对照组分别为(66.30±30.85) pg/mL和(5.29±3.73) mg/L,都明显低于治疗前(P<0.05),同时两组间差异也有统计学意义(P<0.05).结论 血液灌流治疗重症手足口病患儿能有效抑制炎症因子的表达,促进患儿心肺功能的恢复,改善患儿的血气状态.“,”Objective To investigate the clinical effects of hemoperfusion in children with severe hand-foot-mouth disease (HFMD).Methods From September 2009 to September 2015,82 children with HFMD were selected in our hospital as the research subjects and they were then divided into the hemoperfusion treatment group and the control group.The lung function,blood gas analysisand expression of inflammatory cytokines were observed and compared between before and after treatment.Results After treatments,the forced vital capacity (FVC) and the ratio of forced expiration volume in first second to forced vital capacity (FEV1 / FVC) in the two groups showed statistical significance (P <0.05).The FVC and FEV1 / FVC values of the hemoperfusion group after treatments were both significantly higher than those of than the control group (P <0.05).The heart rates and respiratory rates of the two groups were lower than those before treatment.The heart rates and respiratory rates of the hemoperfusion group after treatments were significantly lower than those of the control group (P <0.05).The serum IL-6 and CRP levels of the hemoperfusion group were (58.26 ± 11.84) pg/mL and (1.52 ± 0.65) mg/L after treatment.In the control group the above levels were (66.30 ± 30.85) pg/mL and (5.29 ± 3.73) mg/L.They were significantly lower than those before treatments (P < 0.05),and there was significant difference between two groups (P < 0.05) after treatment.Conclusions Hemoperfusion in children with HFMD showed effective inhabitation of the expression of inflammatory cytokines,improvement of blood gas status and assistance in cardiorespiratory function rehabilitation.
