[目的]了解屏南县流行性脑脊髓膜炎(流脑)带菌状况和免疫抗体水平,预测流脑发病的趋势,为流脑的防控工作提供科学依据。[方法]2009年12月采取分层随机抽样方法采集0~、3~、7~19、≥20岁共4个年龄组健康人群的咽拭子和静脉血,检测脑膜炎奈瑟菌(Nm)IgG抗体。培养采用常规方法,抗体水平测定用ELISA方法。[结果]调查188名健康人群流脑带菌率为零。抗A群Nm抗体阳性率为92.02%,几何平均浓度(GMC)为7.46μg/ml;抗C群抗体阳性率为48.4%,GMC为2.39μg/ml,男女比较抗体浓度和阳性率差异无统计学意义(P>0.05)。不同年龄组A群抗体阳性率差异无统计学意义(P>0.05),C群抗体差异有统计学意义(P<0.05),3~6岁年龄组抗体阳性率最高,0~2岁年龄组最低。A和A+C群流脑疫苗均有接种的A群抗体、C群抗体阳性率均最高,都未接种者阳性率最低,部分无免疫史的人群也检出相应抗体,提示存在Nm自然感染的危险。[结论]屏南县不同年龄组健康人群A群流脑IgG抗体的阳性率维持在较高水平,而C群抗体阳性率较低,应加强流脑疫情监测,普及A+C流脑疫苗的免疫接种,防止流脑的暴发和流行。
[Objective] To understand the prevalence and immunoglobulin level of epidemic cerebrospinal meningitis in Pingnan County and to predict the trend of developing meningococcal meningitis in order to provide a scientific basis for prevention and control of meningitis. [Methods] Throat swabs and venous blood samples from 4 healthy population aged 0 ~, 3 ~, 7 ~ 19, ≥20 years old were collected by stratified random sampling method in December 2009 to detect Neisseria meningitidis ) IgG antibody. The culture was performed by the conventional method and the antibody level was measured by an ELISA method. [Results] The prevalence of tetracycline infection in 188 healthy people was zero. The positive rate of anti-group A Nm antibody was 92.02%, the geometric mean concentration (GMC) was 7.46μg / ml; the positive rate of anti-C group antibody was 48.4% and the GMC was 2.39μg / ml, there was no statistical difference between male and female antibody concentration and positive rate Significance (P> 0.05). There was no significant difference in the antibody positive rate of group A antibody in different age groups (P> 0.05), and the antibody in group C was statistically significant (P <0.05). The antibody positive rate in age group 3 to 6 was the highest, while that in 0-2 age group lowest. A group and A + C group meningitis vaccine were vaccinated group A antibody, C group antibody positive rate was highest, were not vaccinated the lowest positive rate, some non-immune history of the population also detected the corresponding antibodies, suggesting the existence of natural infection with Nm danger. [Conclusion] The positive rate of IgG antibodies in group A meningitis of healthy population in different age groups in Pingnan County is relatively high while the positive rate of antibody in group C is low. Monitoring of meningococcal disease should be strengthened to popularize A + C meningitis vaccine Immunization to prevent outbreaks and epidemics of meningitis.
