目的探讨妊娠期肝内胆汁淤积症(ICP)患者胎盘组织瘦素mRNA的表达及其临床意义。方法采用实时荧光定量(RTQ-PCR)法检测2014年3月至2016年3月安庆市立医院收治的30例ICP孕妇和12例正常妊娠孕妇胎盘瘦素mRNA表达水平,其中ICP患者按不同临床分度、不同孕周分类。结果 1瘦素mRNA在42例胎盘组织中均有表达;215例轻度ICP组瘦素mRNA水平(0.220 6±0.170 1)U/μL、15例重度ICP组(0.176 9±0.147 0)U/μL及12例正常妊娠组的瘦素mRNA水平(0.154 5±0.177 3)U/μL相比,差异无统计学意义(P>0.05);313例ICP足月产组瘦素mRNA水平(0.285 5±0.173 4)U/μL、17例ICP早产组为(0.132 5±0.108 7)U/μL、正常妊娠组为(0.154 5±0.177 3)U/μL,3组比较差异有统计学意义(F=4.121,P=0.024),但两两比较,ICP早产组与正常妊娠组差异无统计意义(P>0.05)。结论瘦素在足月产ICP患者中呈高表达趋势,缺氧可能是主要原因,胎盘组织中瘦素mRNA表达水平在一定程度上反应了胎盘功能,可能参与ICP发病。
Objective To investigate the expression of leptin mRNA in placenta of patients with intrahepatic cholestasis of pregnancy (ICP) and its clinical significance. Methods The expression of placental leptin mRNA in 30 pregnant women and 12 normal pregnant women who were admitted to Anqing Municipal Hospital from March 2014 to March 2016 was detected by real-time fluorescence quantitative PCR (RTQ-PCR). However, no significant difference was found between ICP preterm group and normal pregnancy group (P> 0.05). Conclusion Leptin is highly expressed in full-term ICP patients. Hypoxia may be the main reason. The expression of leptin mRNA in placenta may reflect placental function to a certain degree, which may be involved in the pathogenesis of ICP.
