高频超声在新生儿睾丸扭转诊断及随访中的临床价值

目的:评估高频超声在新生儿睾丸扭转诊断及随访中的临床价值。方法:回顾性分析26例新生儿睾丸扭转病例的超声表现及预后情况。结果:超声诊断睾丸扭转25例,有2例为双侧发生,共27侧。其中3侧睾丸存活,表现为睾丸形态、回声尚正常,血流明显减少但阻力指数(RI)增高;另外24侧睾丸坏死,可见睾丸位置不同程度抬高,回声增高、减低或不均匀,22侧出现形态改变,睾丸内血流信号均消失。超声误诊1例仅表现为睾丸内血流稍增多,手术予保留。术后经过2个月~3年的超声随访,4侧存活的睾丸中3侧生长正常,1侧形态小,所有病例的健侧睾丸生长正常。结论:高频超声诊断新生儿睾丸扭转准确率高,同时可有效评估术后睾丸的生长发育情况。 Objective: To evaluate the clinical value of high frequency ultrasound in the diagnosis and follow-up of testicular torsion in newborns. Methods: The ultrasound findings and prognosis of 26 cases of neonatal testicular torsion were analyzed retrospectively. Results: Ultrasound diagnosis of 25 cases of testicular torsion, 2 cases occurred bilateral, a total of 27 sides. Testicular testicular survived on 3 sides showed normal testicular morphology with normal blood flow and markedly decreased blood flow but increased RI. Side morphological changes, testicular blood flow signals are disappeared. One case of ultrasound misdiagnosis showed only a slight increase in blood flow in the testes, surgery to retain. After 2 months to 3 years after the ultrasound follow-up, 4 survived testes in 3 normal growth, 1 side morphology is small, all cases of normal testicular growth. Conclusion: High-frequency ultrasound diagnosis of neonatal testicular torsion accuracy, and can effectively evaluate postoperative testicular growth and development.
