患者男,23岁,烦渴多尿16年,精神异常14小时,于1991年10月5日上午9时来本院急诊.患者于7岁时因烦渴、多尿在上海某医院确诊为尿崩症,一直用长效尿崩停治疗,开始剂量为0.3mg/次,根据尿量隔2～3天或7～9天肌注1次.17岁开始改为 0.4mg/次.曾于10岁和16岁时两次注射尿崩停后出现神志不清,四肢抽动,每次持续3～4小时.本次于10月1日晚和10月4日晚6时各肌注长效尿崩停针0.4mg(上海生物化学制品厂生产).末次注射1小时后出现胡言乱语,躁动.曾服安定片5mg.5小时后患者出现两眼凝视,呼之不应,四肢抖动,14小时后送到本院急诊室.体检:胡言乱语,颈无抵抗,心率86次/分,律齐,心音强,两肺呼吸音清,腹软,肝脾未及.双侧巴氏征阴性,双下肢膝反射正常.眼底视盘清,视乳头无水肿.实验室检查:Hb123g/L,WBC11.8×10~9/L,N 0.81,L 0.10,MO. 03.
