目的:研究实施医院-社区一体化管理对于糖尿病患者血糖控制的影响.方法:选取2016年2月至2017年2月我院收治的糖尿病患者100例作为研究对象,将其分为两组,对照组患者开展常规管理模式,研究组则在对照组基础上开展医院-社区一体化管理模式,观察比较两组效果.结果:研究组患者护理后糖化血红蛋白、空腹血糖、低密度脂蛋白、总胆固醇、体重与体质指数显著优于对照组,P<0.05,比较具备统计学意义.结论:实施医院-社区一体化管理有利于糖尿病患者的血糖控制,促进患者康复,可以进行大力推广.
