目的:基于颈椎骨龄定量分期法(quantitative cervical vertebral maturation,QCVM),比较应用CICE-下颌前移器(CICE:comfortable,invisible,concise,efficient)和活动式斜面导板,对下颌骨发育不足的安氏Ⅱ类Ⅰ分类错畸形矫形治疗的疗效。方法:选取武汉协和医院口腔医学中心正畸科安氏Ⅱ类Ⅰ分类,QCVM Ⅱ期患者48例,年龄10.5~14.1岁(平均年龄12.7岁,男21例,女27例)。所有患者在固定矫治前行Ⅰ期导下颌向前颌骨矫治,分别应用CICE-下颌前移器(CICE组)和活动式斜面导板(斜导组),每组24例。两组患者均导下颌向前6个月,并保持3个月。对比分析矫形治疗前后的头影测量X线片。结果:两组患者矫形治疗后侧貌均明显改善,前牙覆覆盖正常,磨牙中性关系。在前牙和磨牙关系的改善中,CICE组的骨性变化比例(70.09%、85.67%)均大于斜导组(61.90%、69.48%),同时CICE组切牙的牙根吸收情况明显好于斜导组(P<0.05)。结论:CICE-下颌前移器在治疗安氏Ⅱ类Ⅰ分类错畸形中与活动式斜面导板相比,具有疗效稳定、舒适美观的优势,同时也可规避下前牙唇倾、切牙牙根吸收等并发症。
Objective: To compare the cervical spine bone age (QCVM) with CICE: CICE: comfortable, invisible, concise, efficient and active bevel guide Curative effect of Class Ⅱ Ⅰ malocclusion treatment. Methods: Orthodontic department of Wuhan Union Medical College Hospital orthodontic Class Ⅱ Division Ⅰ Ⅰ, 48 patients with QCVM Ⅱ stage, aged 10.5 to 14.1 years (mean age 12.7 years, 21 males and 27 females). All patients underwent primary mandibular anterior maxillary orthopedic correction before fixation, and 24 patients in each group were treated with CICE-CMP and CSP. Patients in both groups guided mandibular anterior 6 months and maintained for 3 months. Compare and analyze the cephalometric X-ray before and after orthopedic treatment. Results: After orthopedic treatment, the lateral appearances were significantly improved in both groups. The anterior cover was covered properly and the molar relationship was normal. In the improvement of the relationship between the anterior teeth and the molars, the skeletal changes in CICE group (70.09%, 85.67%) were greater than those in the bevel group (61.90%, 69.48%). The root resorption in the CICE group was significantly better than that in the CICE group Lead group (P <0.05). Conclusion: CICE-mandibular advancer has the advantages of stable efficacy, comfortable and beautiful appearance compared with the movable bevel guide in the treatment of Class Ⅱ division Ⅰ malocclusion. At the same time, CICE-mandibular advancement device can avoid the inclination and incisor root of lower anterior teeth Absorption and other complications.
