下颌骨肿瘤切除术后重建的目的,是恢复形态、咀嚼、发音和吞咽功能。应用现代显微外科修复技术,坚固内固定技术和骨融合种植体技术,下颌骨缺损修复的质量已大大提高。在恢复下颌骨形态时,骨瓣的准确就位十分重要。过去常用的方法包括在下颌骨切除前放置修复板,以颌间固定、外固定和牙弓夹板维持上、下颌骨问的立体关系。但这些方法并不十分可靠,有时骨瓣就位不理想。患者需作二次修整手术。 作者回顾利用显微外科技术进行下颌骨重建的经验,介绍一种使骨瓣和髁状突准确就位的方法。
The purpose of reconstruction after mandibular tumor resection is to restore the morphology, chewing, pronunciation and swallowing function. The application of modern microsurgical repair technique, rigid internal fixation technique and bone fusion implant technique has greatly improved the quality of mandibular defect repair. In the restoration of mandibular morphology, the exact position of the bone flap is very important. Traditionally used methods include placement of a prosthetic plate prior to mandibular resection to maintain the three-dimensional relationship between the upper and lower jaws with intermaxillary fixation, external fixation, and dental arch splints. However, these methods are not very reliable, sometimes not ideal bone in place. Patients need to make a second dressing surgery. The authors review the experience of using microsurgery for mandibular reconstruction and describe a method for accurately positioning the bone flap and condyle.
