目的探讨急性砷化氢中毒对心脏的影响。方法以42例急性砷化氢中毒患者作为病例组(轻度中毒组20例、重度中毒组22例),另设健康对照组42例,以受试者心电图和血清生化指标[肌酸磷酸激酶、天冬氨酸转氨酶、α-羟丁酸脱氢酶、乳酸脱氢酶,以及血红蛋白(Hb)和总蛋白(TP)]是否异常为评价标准。结果病例组心电图异常率为73.81%,轻度中毒组的心电图异常率明显低于重度中毒组,差异有统计学意义(P<0.05);轻度、重度中毒组心肌酶、TP以及Hb与对照组比较,差异均有统计学意义(P<0.05);病例组中正常心电图组与异常心电图组的心肌酶谱比较,差异有统计学意义(P<0.05)。结论心电图和血清生化指标可作为急性砷化氢中毒心脏损害的观察指标。
Objective To investigate the impact of acute arsine poisoning on the heart. Methods Forty-two patients with acute arsine poisoning were selected as the case group (20 in mild poisoning group and 22 in severe poisoning group). Another 42 healthy controls were enrolled in this study. The ECG and serum biochemical indicators [creatine phosphokinase , Aspartate aminotransferase, α-hydroxybutyrate dehydrogenase, lactate dehydrogenase, and hemoglobin (Hb) and total protein (TP)] were evaluated as abnormal. Results The abnormal rate of electrocardiogram in case group was 73.81%. The abnormal rate of electrocardiogram in mild poisoning group was significantly lower than that in severe poisoning group (P <0.05). The levels of myocardial enzymes, TP and Hb in mild and severe poisoning group were significantly lower than those in severe poisoning group (P <0.05). There was significant difference in myocardial enzymes between the normal ECG group and the abnormal ECG group in the case group (P <0.05). Conclusion Electrocardiogram and serum biochemical indicators can be used as an indicator of cardiac damage caused by acute arsine poisoning.
