目的观测中药膏剂“CYG”对大面积全厚皮肤缺损创面的促愈效果并探讨其促愈机制。方法贵州小型猪7只,每只背部脊柱两侧近脊柱均有全厚皮肤缺损创面(大小10.5cm×8.5cm)各2个,共28个创面。右侧的14个创面用中药膏剂“CYG”治疗作实验组,同体左侧创面作对照。观察伤后7周内各创面愈合时间、愈合率及愈合过程病理变化,并通过S-P法层粘蛋白(Laminin)免疫组化染色观测其肉芽组织内血管生成变化,免疫组化染色、RT-PCR法检测转化生长因子-β1(transforming growth factor beta one,TGF-β1)表达。结果伤后7~44d,治疗组与对照组创面平均残留面积及愈合残留面积百分率比较,差异有统计学意义(P<0.01),治疗组于伤后32~34d完全愈合,较对照组提前10~12d,且创面质地细腻、平整。愈合过程中治疗组肉芽组织生长早且旺盛,肉芽组织中Laminin、成纤维细胞、毛细血管及胶原含量丰富;治疗组各时相点创面TGF-β1表达较对照组低,同组TGF-β1表达:中期(伤后15~35d)>早期(伤后7~14d)>晚期(伤后66~49d)。结论中药膏剂“CYG”对大面积全厚皮肤缺损创面有良好促愈作用,且创面愈合质量高,并可能对创面瘢痕形成具抑制作用。
Objective To observe the effect of traditional Chinese medicine “CYG” on the wound healing of large-area full-thickness skin defects and explore its healing mechanism. Methods There were 7 miniature pigs in Guizhou. There were 2 wounds of full-thickness skin defect (10.5cm x 8.5cm in size) on each side of the spine. There were 28 wounds. The 14 wounds on the right side were treated with traditional Chinese medicine “CYG” as the experimental group and the left side of the same body as the control. The wound healing time, healing rate, and pathological changes of wound healing were observed within 7 weeks after injury. The changes of angiogenesis in granulation tissue were observed by laminin immunohistochemical staining with SP method, immunohistochemical staining and RT-PCR. The expression of transforming growth factor beta 1 (TGF-β1) was detected. Results 7 to 44 days after injury, the average residual area and the percentage of residual area after healing in the treatment group and the control group were statistically significant (P<0.01). The treatment group completely healed 32 to 34 days after the injury, which was 10% earlier than the control group. ~12 days, and the texture of the wound is delicate and smooth. During the healing process, granulation tissue in the treatment group grew early and flourished. Laminin, fibroblasts, capillaries, and collagen were abundant in the granulation tissue. The expression of TGF-β1 in the wound was lower in the treatment group than in the control group. The expression of TGF-β1 in the same group. : Intermediate (15-35 days after injury)> Early (7-14 days after injury)> Late (66-49 days after injury). Conclusion The traditional Chinese medicine plaster “CYG” has a good healing effect on large area full-thickness skin defect wounds, and the wound healing quality is high, and it may have an inhibitory effect on wound scar formation.
