目的通过观察清热益气通络方对糖尿病肾病(DN)大鼠肾小球足细胞表面蛋白(Podocalyxin)、结蛋白(Desmin)表达的影响,探索该方治疗DN蛋白尿的机制。方法将SD大鼠随机分为正常组(N)和糖尿病肾病造模组(采用单侧肾切除腹腔注射链脲佐菌素的方法建立大鼠糖尿病肾病模型)。将造模成功的大鼠再分为模型对照组(M)、中药组(Q)和厄贝沙坦组(I)。干预治疗12周后处死大鼠,检测各组大鼠24h微量白蛋白(umAlb)、24h尿蛋白量(uPro)、血肌酐(SCr)。免疫组织化学染色的方法观察肾小球Podocalyxin、Desmin蛋白的表达。光镜及电镜观察肾小球结构。结果 Q组大鼠24h umAlb、SCr均低于M组(P<0.05),24h uPro显著低于M组(P<0.01)。与N组相比Q组大鼠肾小球Podocalyxin表达显著降低(P<0.01),但明显高于M组(P<0.05);Q组大鼠肾小球Desmin表达高于N组(P<0.01),低于M组(P<0.05)。结论清热益气通络中药能减弱足细胞上皮间充质转分化(EMT),从而降低DN大鼠蛋白尿,改善肾功能。
Objective To observe the effect of “Qingre Yiqi Tongluo Prescription” on expression of Podocalyxin and Desmin in glomerular podocyte of diabetic nephropathy (DN) rats and explore the mechanism of this recipe in treating DN proteinuria. Methods SD rats were randomly divided into normal group (N) and diabetic nephropathy model group (diabetic nephropathy model was established by intraperitoneal injection of streptozotocin). The successful rats were divided into model control group (M), Chinese medicine group (Q) and irbesartan group (I). After intervention for 12 weeks, the rats were sacrificed and the 24hours of umAlb, uPro and SCr were measured. The expression of Podocalyxin and Desmin protein in glomeruli was observed by immunohistochemical staining. Light microscopy and electron microscopy glomerular structure. Results 24h umAlb and SCr in Q group were significantly lower than those in M group (P <0.05), 24h uPro was significantly lower than that in M group (P <0.01). Compared with N group, the expression of Podocalyxin in glomerulus in Q group was significantly decreased (P <0.01), but significantly higher than that in M group (P <0.05). Desmin expression in glomerular Q group was higher than that in N group (P < 0.01), lower than M group (P <0.05). Conclusion: Qingre Yiqi Tongluo Chinese herbs can reduce the epithelial mesenchymal transition (EMT) of podocytes, thereby reducing proteinuria and improving renal function in DN rats.
