目的 探讨生物型人工全髋关节置换术治疗Crowe Ⅳ型髋关节发育不良的临床疗效.方法 100例Crowe Ⅳ型髋关节发育不良患者均给予生物型人工全髋关节置换术治疗,将44例术中接受转子下短缩截骨患者设为甲组,56例未截骨者作为乙组,比较两组手术相关指标、治疗前后髋关节Harris评分及双下肢长度差值.结果 甲组患者手术时间显著长与乙组(P<0.01),术中出血量及术后引流量显著多于乙组(P<0.01).治疗后两组髋关节Harris评分均显著高于治疗前(P<0.01).治疗前甲组双下肢长度差值显著大于乙组(P<0.01),治疗后两组差值均显著缩小(P<0.01),且两组间比较差异无统计学意义(P<0.01).结论 采用生物型人工全髋关节置换术治疗Crowe Ⅳ型髋关节发育不良患者,有利于提高关节功能,恢复肢体长度,应综合考虑患者情况决定是否行转子下短缩截骨手术,以提高治疗效果.“,”Objective To investigate the clinical effect of biological type THA in the treatment of Crowe type IV DDH. Method 100 cases with Crowe type Ⅳ DDH were all given biological type THA, 44 cases underwent subtrochanteric shortening osteotomy were seen as group A, 56 cases without osteotomy were seen as group B,compare the surgical related indicators, Harris scores, the difference of both lower extremities length of the two groups. Results The operation time of group A was significantly longer than that of group B (P<0.01), the peroperative bleeding and drainage were significantly more than those of group B (P<0.01). After treatment, Harris scores of the two groups were all significantly higher than those before treatment (P<0.01). Before treatment, the difference of both lower extremities length in group A was significantly larger than that in group B (P<0.01),after treatment, the difference of the two groups was significantly lessened (P<0.01), and the difference between the two groups had no statistic significance (P <0.01). Conclusion The application of biological type THA in the treatment of Crowe type IV DDH can improve the joint function and recover the limb length of patients. The subtrochanteric shortening osteotomy should be considered according to the condition of patients in order to achieve a satisfactory therapeutic effect.
