目的:了解农村贫困地区卫生人力资源现状,发现其中存在的问题,并提出相应的建议。方法:抽取全国6个省的13个样本县,调查其中的51家县级医疗机构,121家乡镇卫生院;分别采用了现场调查和函调以及问卷调查、个人深入访谈、专题小组讨论等方法。结果:我国农村贫困地区卫生人员总量不足;卫生队伍结构不合理、整体素质不高;贫困地区卫生技术人员的收入水平与其创造的价值不相匹配;卫生人力资源配置“缺编”和“空编”问题并存;乡村医生的身份和职业定位不清。建议:构建适应贫困地区卫生人力开发的医学教育体系;继续以农村贫困地区适宜性卫生人才为主线,引进、培养和使用人才;通过定向培养、职称评定、工资待遇等政策,调整农村卫生人员队伍结构;营造良好的农村卫生人才环境,稳定农村贫困地区的卫生人才。
Objective: To understand the status quo of health human resources in rural poverty-stricken areas, find out the existing problems and put forward corresponding suggestions. Methods: Thirteen sample counties in 6 provinces of China were selected to investigate 51 county-level medical institutions and 121 township hospitals. The methods of on-the-spot investigation and correspondence survey, questionnaire survey, personal in-depth interviews and panel discussions were used . Results: The total number of health workers in rural poverty-stricken areas is not enough; the structure of health workers is irrational and the overall quality is not high; the income level of health technicians in impoverished areas does not match the value created by them; “Empty series ” problems coexist; the identity and professional orientation of rural doctors unclear. Suggestion: Construct a medical education system that adapts to the development of human resources in poor areas; continue to introduce, train and use qualified health personnel in poor rural areas as the main line; and adjust the ranks of rural health workers through policies such as targeted cultivation, professional title assessment and salary treatment Structure; create a good environment for rural health professionals, and stabilize rural health personnel in poor areas.
