目的探讨下肢深静脉血栓CDFI(彩色多普勒血流显像)筛查在诊治地震伤员中的应用。方法对23例汶川大地震伤员进行常规下肢深静脉血管探查,分析其声像图,对检出下肢深静脉血栓患者及时进行抗凝、溶栓等处理,并追踪观察治疗效果。结果23例地震伤员中,检出下肢深静脉血栓5例,总发生率21.7%;近心端静脉血栓1例,占20%,远心端静脉血栓4例,占80%;较易受累的静脉为小腿肌间静脉丛和交通支静脉;经抗凝、溶栓后血栓完全复通者3例,缩小、机化者2例。结论地震伤员下肢深静脉血栓形成发生率较高,CDFI筛查可起到早期发现、及时干预和积极预防的作用。
Objective To investigate the application of deep venous thrombosis (CDFI) screening of deep venous thrombosis in diagnosis and treatment of earthquake wounded. Methods Twenty-three cases of deep Wenchuan earthquake were examined by conventional deep venous blood vessels of the lower extremities. The sonographic findings were analyzed. Anticoagulation and thrombolysis were performed in patients with deep venous thrombosis of the lower extremities in a timely manner. The therapeutic effect was tracked. Results Among the 23 earthquake victims, 5 cases of deep venous thrombosis of the lower extremities were found, with a total incidence of 21.7%. One case of proximal venous thrombosis (20%) and 4 cases of distal venous thrombosis (80%) were more likely to be affected Intravenous venous calf muscle and traffic branch vein; by anticoagulant, thrombolytic thrombosis completely recanalization in 3 cases, narrowing, machine in 2 cases. Conclusion The incidence of deep venous thrombosis in lower extremities in earthquake victims is high. CDFI screening can play an important role in early detection, timely intervention and active prevention.
