目的:观察以疏风清热、活血化瘀立法治疗过敏性紫癜(HSP)前后,外周血转化生长因子β(TGF-β)和白细胞介素6(IL-6)在纯真CD4+T细胞(Naive T)分化为辅助性T细胞17(Th17)和调节性T细胞(Treg)的过程中的变化。方法:使用流式细胞仪检测36例住院患者治疗前后和健康对照组外周血Th17、Treg的水平,使用实时荧光定量PCR仪检测TGF-β、IL-6和转录因子ROR-γt的水平,比较其差异。结果:(1)HSP患者治疗后与治疗前(急性期)相比较,IL-6和Treg的水平升高(P<0.05),Th17和ROR-γt的水平降低(P<0.05),TGF-β的水平无明显差异(P>0.05);(2)HSP患者治疗前与健康对照组相比较,Th17和ROR-γt的水平明显升高(P<0.05),IL-6和Treg的水平降低(P<0.05),TGF-β的水平无明显差异(P>0.05);(3)HSP患者治疗后与健康对照组相比较,IL-6和Treg的水平升高(P<0.05),Th17和ROR-γt的水平降低(P<0.05),TGF-β的水平无明显差异(P>0.05)。结论:以疏风清热、活血化瘀立法治疗过敏性紫癜,能明显干预患者的外周血TGF-β、IL-6、ROR-γt、Th17及Treg的水平,对患者免疫功能的调节作用明确,临床效果显著,极大的提高了临床诊疗能力。
Objective: To observe the effects of Shufeng Qingre, promoting blood circulation and removing blood stasis before and after treatment of Henoch-Schonlein purpura (HSP) in peripheral blood CD4 (superscript +) T cells (Naive T) differentiation into helper T cells 17 (Th17) and regulatory T cells (Tregs). Methods: The levels of Th17 and Treg in peripheral blood of 36 inpatients before and after treatment and healthy controls were detected by flow cytometry. (2) The levels of Th17 and ROR-γt in patients with HSP before treatment were significantly higher than those in healthy controls (P <0.05), while the levels of IL-6 and Treg were decreased (P <0.05). There was no significant difference in the level of TGF-β between the two groups (P> 0.05). (3) The levels of IL-6 and Treg in the HSP patients were significantly higher than those in the healthy controls And ROR-γt (P <0.05), while there was no significant difference in TGF-β (P> 0.05). Conclusion: The treatment of Allergic purpura by Shufeng Qingre and activating blood circulation to remove blood stasis can obviously interfere the level of TGF-β, IL-6, ROR-γt, Th17 and Treg in the peripheral blood of patients, The clinical effect is remarkable, greatly improving the clinical diagnosis and treatment ability.
