CT能谱曲线及碘含量测定在早期胃癌诊断中的应用价值

目的

探讨双源CT能谱曲线及碘含量测定在早期胃癌诊断中的应用价值。

方法

回顾性分析2011年11月至2013年6月间于郑州大学第一附属医院放射科行双源CT双能量扫描检测的21例经胃镜及手术病理证实的早期胃癌及38例正常胃的影像资料,测量胃癌病灶动脉期和静脉期碘浓度,胃正常者的胃小弯侧胃壁碘浓度及同层主动脉碘浓度,比较早期胃癌病灶与正常胃壁标准化碘浓度(NIC)的差异;分析胃癌病灶能谱曲线及正常胃壁胃小弯侧能谱曲线的特征。

结果

早期胃癌病灶动脉期和静脉期NIC与正常胃比较,差异均具有统计学意义(0.21比0.09,P=0.000;0.72比0.26,P=0.000)。早期胃癌及正常胃壁动脉期和静脉期能谱曲线均呈下降型,早期胃癌能谱曲线位于正常胃壁上方,对应的能谱曲线更为陡直;两者于40~70 keV时差异较大;于80~140 keV对应的曲线差异变小。

结论

NIC及能谱曲线走行趋势有助于对早期胃癌进行定性及定量诊断。
