目的:观察胰岛素强化治疗对初诊2型糖尿病(T2DM)患者糖代谢和炎症因子的影响。方法:对106例新诊断的T2DM患者进行为期2 w的胰岛素强化治疗(强化组,n=106),另入选100例同期进行健康体检的正常人群为正常对照组(对照组,n=100),观察治疗前二组研究对象血糖指标(空腹血糖、餐后2h血糖和糖化血红蛋白)和炎症因子(C反应蛋白、肿瘤坏死因子α和白介素-6)的差异,比较强化组治疗前后血糖指标和炎症因子的变化。结果:观察前强化组的血糖指标及炎症因子明显高于对照组(P<0.05),其他基础临床资料差异无统计学意义(P>0.05)。经胰岛素短期强化治疗后,强化组的血糖指标明显改善(P<0.05),炎症因子水平也显著下降(P<0.05)。结论:胰岛素强化治疗可以改善T2DM患者的糖代谢,降低患者的炎症因子的水平。
Objective: To observe the effects of intensive insulin therapy on glucose metabolism and inflammatory factors in newly diagnosed type 2 diabetes mellitus (T2DM). Methods: A total of 106 newly diagnosed T2DM patients underwent 2-w insulin intensive therapy (intensive group, n = 106). Another 100 normal subjects were enrolled as normal control group (n = 100) , The differences of blood glucose (FBG, 2h postprandial blood glucose and glycosylated hemoglobin) and inflammatory cytokines (C-reactive protein, tumor necrosis factor alpha and interleukin-6) in the two groups before treatment were observed and compared before and after treatment Changes in inflammatory cytokines. Results: Before the observation, the blood glucose and inflammatory factors in the intensive group were significantly higher than those in the control group (P <0.05). There was no significant difference in the other basic clinical data (P> 0.05). After insulin short-term intensive treatment, the glycemic index of the intensive group was significantly improved (P <0.05), and the levels of inflammatory cytokines were significantly decreased (P <0.05). Conclusion: Intensive insulin therapy can improve glucose metabolism in patients with T2DM and reduce the level of inflammatory cytokines in patients.
