目的:探讨血清瘦素(LP)、可溶性瘦素受体(SLR)及沉默信息调节因子相关酶3(SIRT3)水平与高血压左室肥厚(HLVH)之间的关系。方法:选择HLVH患者30例(HLVH组)、原发性高血压(EH)患者30例(EH组)以及健康体检者30例(对照组),行心脏彩超检查计算左室质量指数(LVMI),应用酶联免疫吸附法(ELISA)测受试者血清LP、SLR及SIRT3水平,同时检测空腹血糖(FPG)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、尿素氮(BUN)、肌酐(CR)等相关指标。结果:1EH组LVMI、血清LP及SIRT3水平高于对照组,血清SLR水平低于对照组(P<0.05),HLVH组LVMI、血清LP及SIRT3水平高于EH组和对照组,血清SLR水平低于EH组和对照组(P<0.05);2对于不同级别高血压病患者中,随着血压水平升高,患者LVMI及血清LP、SIRT3水平升高,血清SLR水平呈下降趋势(P<0.05);3LVMI与LP、SIRT3呈显著正相关(r=0.716,0.733;P<0.05),与SLR呈显著负相关(r=-0.675,P<0.05),以LVMI为因变量行多元线性逐步回归分析显示:SBP、LP、SLR、SIRT3、同型半胱氨酸(Hcy)进入多元线性回归方程。结论:EH及HLVH患者血清LP、SIRT3浓度升高,SLP浓度下降,并与血压之间有一定的相关性,说明LP抵抗、SIRT3与EH及HLVH的发生和发展密切相关。
Objective: To investigate the relationship between serum leptin (LP), soluble leptin receptor (SLR), SIRT3 and hypertensive left ventricular hypertrophy (HLVH). Methods: Thirty patients with HLVH (HLVH group), 30 patients with essential hypertension (EH) and 30 healthy controls (control group) were enrolled in this study. Left ventricular mass index (LVMI) Serum LP, SLR and SIRT3 levels were measured by enzyme-linked immunosorbent assay (ELISA). Fasting blood glucose (FPG), total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol C, LDL-C, BUN, CR and so on. Results: The levels of LVMI, serum LP and SIRT3 in 1EH group were higher than those in control group, but the level of SLR in serum was lower than that in control group (P <0.05). The levels of LVMI, serum LP and SIRT3 in HLVH group were higher than those in EH group and control group (P <0.05) .2 In patients with different grades of hypertension, the levels of LVMI, serum LP and SIRT3 increased and the level of serum SLR decreased with the rise of blood pressure (P <0.05 ). There was a significant positive correlation between 3LVMI and LP and SIRT3 (r = 0.716,0.733; P <0.05), and negatively correlated with SLR (r = -0.675, P <0.05). Multiple linear stepwise regression Analysis showed that: SBP, LP, SLR, SIRT3, homocysteine (Hcy) into the multiple linear regression equation. CONCLUSIONS: Serum concentrations of LP and SIRT3 are elevated and the concentration of SLP is decreased in patients with EH and HLVH, and there is a certain correlation between them and blood pressure. This indicates that LP resistance and SIRT3 are closely related to the occurrence and development of EH and HLVH.
