目的 总结国产吻合器在食管癌、贲门癌手术中失败的原因及教训。方法 回顾1981年5月至1999年5月1128例国产吻合器的使用情况。结果 导致吻合缺陷或失败共67例发生率为5.94％,其原因有1.吻合器装配失误,使用不当;2.吻合器型号选择不当;3.食管残端荷包缝线距切缘过近或过远;4.吻合器附近组织、异物嵌入抵针座与机身之间。结论 国产吻合器有操作简便、节省时间、吻合确切等优点,其吻合效果并不劣于进口吻合器,而且可大大减少费用,只要正确地使用,可明显减少其并发症的发生。
Objective To summarize the reasons and lessons of the failure of the domestic stapler in esophageal and cardiac surgery. Methods From May 1981 to May 1999 1128 cases of domestic use of stapling. The results lead to anastomotic defects or failure a total of 67 cases were 5.94%, the reason 1. Stitcher assembly errors, improper use; 2. Stapler type improper selection; 3. Esophageal stump purse suture from the edge too close or Too far; 4 stapler near the organization, foreign body embedded between the needle seat and the fuselage. Conclusion The domestic stapler has the advantages of simple operation, time saving, exact anastomosis and so on. The anastomosis effect is not inferior to that of the imported stapler, and can greatly reduce the cost. As long as the correct use is made, the occurrence of complications can be obviously reduced.
